Opp Health and Rehabilitation, LLC sits in Opp, Alabama, and offers skilled rehabilitation and long-term nursing care for people needing extra help in their daily lives, and the building has 197 certified beds with an average of about 166 residents day-to-day, and what stands out is that they accept both Medicaid and Medicare, so folks with different needs and coverage can consider it, and the place gets an overall average rating-nothing too high or too low-when you look at their health inspections and staffing, both which come out average too, and if you check their quality measures, those are just a bit below average but not severely so. Over their last inspections, they've had six deficiencies recorded, with one tied to infection control and several connected to nutrition, especially when it comes to storing food and getting it from proper sources, though nothing has actually caused harm, but inspectors thought there was a potential for harm beyond the minimum, and these standard inspection reports go back over two years, even though checks are supposed to happen every nine to fifteen months. The nurse turnover rate's pretty high, sitting at about 52.7%, which is a thing to keep in mind, and each resident gets around 3.73 hours from the nursing staff each day, so that's about average when compared to other similar places. Ownership goes through a for-profit corporation and is affiliated with NHS Management, and their leadership includes Bryan Baggett, Jeanell Perkins, Lynn Rasco, and Julie Richburg, so several professionals are on board. The facility offers COVID-19 testing, and there's an admissions counselor for folks who want to come and look around, and people say the environment feels caring and friendly, with beautifully decorated common spaces and a home-like feel, plus outdoor courtyards where residents can spend time outside, and inside there's a restaurant-style dining area where residents have meals, reliable Wi-Fi, cable TV in the rooms, laundry and beautician services for personal care, and staff keeps a daily calendar of activities to help residents stay active and involved. Trained nurses, certified aides, and therapists work to make sure support is personalized, and even though the place aims to give people independence and comfort, the record does show areas for improvement, especially in nutrition and inspection timelines, but for many, it offers a solid option for those needing both medical and daily care, and the staff strives for safety and well-being in a setting that tries to be both comfortable and pleasant to live in.

Clean, caring staff; inconsistent communication
I placed my loved one at Opp Health & Rehab and found a beautiful, very clean facility with a warm, welcoming staff - admissions (Samantha and Courtney) made the move smooth and kind. Most nurses and CNAs were knowledgeable, attentive, and went above and beyond; rehab and daytime activities were helpful and comforting. That said, communication and responsiveness can be inconsistent at times, and we saw occasional care lapses (missed meds, hygiene and clothing mix-ups) on certain wings. Overall I felt relieved and would recommend Opp Health & Rehab, but stay involved and report concerns promptly.
