Gassville Nursing Center in Gassville, Arkansas, is a skilled nursing facility that serves seniors who need both short-term rehabilitation and long-term care, and it's got certified beds and is Medicare and Medicaid certified, and even though the official documents say there's a total of 31 deficiencies in inspection reports-like concerns about keeping the place safe and clean, and issues about respecting residents' personal items, and some infection-related notes-the place still has procedures to help keep areas safe, infection-control rules, and policies about dignity for everybody. The center has had inspections, with a standard report in July 2024 listing seven deficiencies and a complaint report in November noting two more, yet its nurse turnover rate sits at 43.2%, which is lower than the state average, and while licensed nurse staffing averages 3.52 nurse hours per resident per day, that's below the state average, so folks should bear this in mind. Eagle Health Systems Inc runs the place with Anthony and Bryan Adams as owners and people like Randy Mcchristian and John Scribner managing things as of mid to late 2024, and the community operates both as a nursing home and as a place where assisted living, memory care, short-term rehab, and respite care happen, with 105 licensed beds in total but also mentions of 42 beds in certain care sections. Residents get three nutritious meals each day, housekeeping and laundry, and the facility offers activities such as parties, outings, and daily engagement, plus it's pet-friendly and has a beauty and barber shop for grooming. Folks who stay here can use Wi-Fi and a resident computer; there's van transportation for doctor's visits, individualized care plans tailored to what each person needs, a secure special care neighborhood for those who need extra supervision, and therapies for physical, occupational, and speech needs both as inpatient or outpatient, with licensed therapists and restorative aides on the staff. Care teams include internal medicine practitioners, psychiatrists, and psychoanalysts, and there are services for IV therapy, wound and pain management, plus medication help. Families can count on regular memory care and personal care services. The center says it wants to keep the environment comfortable, clean, and safe, and works to honor residents' dignity and rights, but there is a record of past problems and ongoing efforts to improve, which is something people will want to review. Guests and families can expect long-term and short-term care, with a focus-at least stated-on respect, safety, and a variety of activity programs to help keep residents active. The facility operates under leadership that has come in recently, and it participates in local economic and community development through the South Shore Foundation. With a 3.0 star rating from ten reviews, Gassville Nursing Center stands as one of several senior living options in the area, offering both professional medical attention and supportive services for seniors.

Caring staff but safety concerns
I found the place clean, home-like, and staffed by caring, knowledgeable people - management is hands-on and genuinely responsive. Activities, therapy and a lending library kept residents engaged, and visitors can dine with loved ones; food was decent. I was concerned by chronic understaffing that led to medication delays and by reports of infections/COVID outbreaks and other safety/neglect incidents. Overall my experience was mixed: loving, compassionate care from staff, but weigh the serious staffing and safety concerns before deciding.
