Summit Health and Rehab Center sits at 506 N Long Ave in Taylor, Arkansas, and gives a wide range of skilled nursing, rehabilitation, and elder care services for people from across the state, and with roughly 50 to 99 staff, the center covers long-term and short-term care for those who need it, whether recovering from a hospital stay or needing ongoing support, and the staff, which includes skilled licensed nurses, therapists, and caregivers, gives care around the clock, seven days a week, using personalized care plans for each resident because people need different levels of support, from full nursing care for very frail or dependent folks, to specialized help like speech therapy, physical therapy, occupational therapy, stroke recovery, IV antibiotic therapy, nutritional counseling, wound care, dentistry, podiatry, and respiratory therapy for those with more complex needs, and, of course, the staff helps with everyday needs, homemaking, personal care, and they provide medical transportation for appointments outside the facility when needed. The center also focuses on holistic wellness, meaning they make plans based on the whole person, and they offer daily activities, recreational therapy, and community events to encourage friendship, joy, and fulfillment, because people stay healthier and happier when they feel part of a community, and Summit also encourages resident and family involvement with both a resident council and a family council, so families can stay connected and know what's happening. Summit Health and Rehab Center works under a for-profit Limited Liability Company structure, specializes in orthopedic rehabilitation, comprehensive post-acute care, respite care if a caregiver needs a break, hospice for end-of-life care, and inpatient rehabilitation, so families can turn to them for many needs as loved ones age or recover from illness or injury. The center keeps a modern, comfortable, and safe environment and focuses on each individual's comfort, offering 24/7 nursing care, on-site laboratory and X-ray services, optometry services, and more, with care quality ratings that have shown steady improvement, reaching an A-minus overall in recent inspections, and nurse quality ratings from mostly A to B+ over time, so you can expect a good standard of medical supervision, and the occupancy rate is around 75%, meaning there's room for new residents from time to time. Summit Health and Rehab Center maintains a website where you can learn more or get in touch if you have questions, and they've shown in the past, like with their A+ grades in 2019 and 2020, that they work to keep standards high and well-staffed, all while giving attention to each person's needs in a setting that aims to promote the best health and comfort possible for elders.

Excellent leadership, serious safety concerns
I had mixed experiences: leadership is excellent and some staff were caring and very helpful, but staffing is inconsistent. I witnessed unsafe, abusive handling (residents jolted in slings) and medication mistakes - I can't recommend this place until those serious safety issues are fixed.
