Lakewood Post Acute and Rehabilitation, located at 7395 W Eastman Pl in Lakewood, Colorado, is a skilled nursing facility with 108 certified beds and an average of 19 residents per day, so there's a sense that it's a bit quieter and more manageable than some larger places, and it's been designed as a home-like environment which helps many feel comfortable and safe, and with a focus on short-term rehab after surgeries or medical events you'll find the therapy services are run by their own team of therapists, and the staff say they support both short-term rehab and longer-term care needs depending on what each person needs, sometimes day-to-day and sometimes for the long-haul, as the place gets residents back on their feet with programs for independence, so to speak, and a multidisciplinary team works on customized care plans, so there's some coordination involved between nursing, therapy, and other clinical folks, and since nursing care is available round the clock with an average of 7.00 nurse hours per resident per day, you'll always see someone checking in, although with a nurse turnover rate of 65.4%, lots of faces come and go, and as with many facilities, inspectors have noted several issues, with 41 documented deficiencies in reports, including ones for lacking proper infection control programs and not giving staff all the training about dementia care or voicing concerns about abuse or neglect, so those are things to consider, but in terms of services, they provide diabetes management, IV therapy, orthopedic care, respite, hospice, and dementia care, and there's an activities department that plans weekly outings and daily classes to keep spirits up and minds busy, and social hours are organized so residents get a chance to mingle, and the facility, affiliated with Pacs Group and part of CHCA District I, strives to have a compassionate team and personalized healthcare, keeping things organized and clean, and offers admissions and support with help from a dedicated staff, although you should know front desk hours are 7 am to 7 pm every day, and there's a 24/7 visitation policy which makes it easy for families to drop by as needed, and while the facility says it's committed to high-quality, person-centered care, like many places, it has both strengths and ongoing challenges, and for more details you can always visit their website.

Excellent rehab, serious safety concerns
I found the facility clean and modern with excellent PT/OT and several CNAs and nurses who went above and beyond. However staffing and communication were inconsistent - long call-button waits, missed or delayed meds and therapies, and poor weekend coverage. I witnessed neglectful hygiene and rough handling, catheter-related infections, inadequate pain management, and medication errors that led to ER visits. Administration and billing were often unhelpful, family updates and doctor contact were inconsistent, and some staff were rude or confrontational. Overall my experience was mixed: top-notch rehab and a few outstanding caregivers, but serious safety, staffing and management lapses - use caution.
