The Center at Park West is a nursing home located in Pueblo, Colorado, and its main focus is helping seniors who need skilled nursing, long-term care, or rehabilitation after a hospital stay before going home, and they've built a place with 80 private suites where every room has a private bathroom and shower, and they put a lot of attention on comfort and privacy since everything's pretty modern and built to support folks who need daily care. The staff follow physician-driven care plans, so each person gets care based on what a doctor says will help them most, and that includes skilled nursing services, physical, occupational, and speech therapy, as well as psychological care, family involvement, and planning for when someone can safely go home, and they take care to encourage residents to be as independent as possible. The rehabilitation program covers things like walking, building strength, balance, managing steps, and regaining motion, so folks get the therapy they need to feel better and work toward their highest level of function, and the staff also help with all the medical side like routine blood draws, giving out medicine, x-rays, and other health checks. They also put effort into meals, with food made by an executive chef, and offer social support and psychological care to help people recover. Their amenities focus on comfort and convenience, with a facility that's about 68,500 square feet, and all care is managed under the rules of Colorado's skilled nursing facility license and operated by Veritas Management Group. The Center at Park West provides over 154 services covering nursing home care, rehab, social services, medical services, and nutrition, and is rated 3.4 from 57 reviews, so while it serves many needs, some people have had mixed experiences. The Center at Park West isn't the fanciest place around but it is fully equipped to care for seniors who need round-the-clock skilled nursing, rehab, or temporary care after hospital stays, and their focus is making sure everyone gets a care plan that fits their needs and keeps them as safe and independent as possible.

Clean facility but dangerously understaffed
I had a mixed experience. The facility is clean and well-appointed, and many CNAs, nurses and therapists (Michelle and a few others) were excellent, compassionate, and helpful. But chronic understaffing and poor management led to long call-button waits (15-20+ minutes), late or missed meds/meals (including insulin/pain meds errors), poor communication, bed-sore risk and unsafe care, plus chaotic discharge/insurance handling. Because of those safety and consistency issues, I cannot recommend this facility.
