Riverwalk Post Acute and Rehabilitation sits over on 1610 Scranton Ave. in Pueblo, Colorado, and has been serving the area since 1987 with both Medicare and Medicaid certification, so if you or someone you know needs skilled nursing or higher level support, this 60-bed facility does have round-the-clock nursing care, short-term rehab, and even memory care for people with Alzheimer's or dementia, and while nobody likes talking about problems, the place has 14 recorded deficiencies and hasn't been fined, which people might want to consider. Residents and families get counseling, and those coming from hospitals or needing extra help can expect both post-acute care and long-term nursing, since the staff uses an interdisciplinary approach including nurses, physicians, families, and social services to make sure therapy and treatment fit each person's needs, and they try to help patients get back to their homes and daily routines when possible. Folks living here can stay in private or semi-private rooms, the place is modern with state-of-the-art facilities for therapy, and there's a dedicated in-house therapy team for both hands-on and equipment-based rehab work, which includes physical, occupational, and speech therapy, so if someone needs to recover strength and mobility, the options range from rehab programs and outpatient therapy to permanent care. The grounds have well-kept landscaping, and inside there's housekeeping, laundry services, good food, and a schedule full of activities and excursions meant to keep folks involved and active, whether it's for short-term rehab or a longer-term arrangement. The community carries an average rating of 7.8 out of 10 in Pueblo, ranks as the tenth highest in the city, and real people have given it a 4.1 out of 5 based on reviews, and since it's part of the wider Ensign Services network, there's some extra support there, and someone interested can always find more at their riverwalkrehabilitation.com website.

Misleading photos, unsafe care; avoid
I had a mostly negative experience and wouldn't even give this place one star. The website photos were misleading - it isn't as nice as advertised and not really near the river - and I heard gunshots nearby. Communication and safety were unacceptable: doctors/nurses gave poor or no updates, staff missed patient alarms, a CNA incident felt like elder abuse, and personal items (cell phone, pajamas) went missing after discharge; my family wasn't told about the death in a timely way. There were bright spots - some nurses and therapists were fantastic, rooms are spacious, walks are easy and pleasant, and meals/therapy were good at times - but the serious lapses make me strongly discourage sending your loved ones here.
