    Excellent nurses, overall negligent care

I had mixed experiences. The rehab team and some nurses were excellent and helped my recovery, but many CNAs didn't follow laws or protocols and management was disorganized. Rounds were often delayed, staff frequently didn't respond, and neglected medical care allowed UTIs to turn septic. Meals were tiny and unsatisfying - overall care quality was poor and I would not return.

About Columbine Commons Health & Rehab Facility

Columbine Commons Health & Rehab Facility sits on Main Street in Windsor, Colorado, as part of the Columbine Health Systems campus, and operates under the direction of Mr. Palmer Withrow as administrator and Ms. Lynann Moses as director, having served the community for 11 years now, and although it isn't BBB accredited, it's managed to achieve an A+ rating. This skilled nursing and rehabilitation facility, known for the Lakes at Columbine Commons Health & Rehab building, is modern, has 30 beds, and holds a five-star CMS quality rating from Medicare and Medicaid which isn't a small feat, and it also managed to keep 90% occupancy or higher since opening and hit full census by July after its first opening, which shows steady demand. Residents here find on-site care from over 160 CHS employees, without relying on agency staff, which can provide a feeling of consistency. Columbine Commons offers both short-term rehabilitation services-daily from 8 AM to 5 PM-and long-term care for people coming from the hospital or those with ongoing health needs, and it's recognized for thoughtful skilled nursing services including recuperation after surgery, diabetic and incontinence care, non-ambulatory help, medication management, and round-the-clock licensed nursing care, plus hospice and palliative care for those who need support near the end of life. The facility stands near major medical centers such as Northern Colorado Medical Center, Poudre Valley, and McKee Medical Center, and also sits next to a physician, lab, and x-ray clinic, which makes regular medical check-ups and urgent support easier for residents.

The campus also includes Westwood Patio Homes, the Windsor Independent Living, and Columbine Commons Assisted Living, plus two skilled nursing and rehab facilities called Mountains and Lakes, with the Lakes facility celebrating its first anniversary just recently. For those needing help with daily living, assisted living services are available, which feature personalized care plans to promote independence, private rooms or apartments with walk-in closets, kitchenettes, private bathrooms, and living spaces with cozy furnishings, large windows, and space for personal touches. Residents enjoy nutritious meals and restaurant-style dining, daily social activities, fitness classes, on-site rehabilitation, devotional services, exercise opportunities, outings, housekeeping, laundry, emergency services, and scheduled van transportation. Social and recreational activities fill the calendar, from indoor common areas where friends gather, to walking paths and outdoor courtyards for fresh air and exercise, with staff encouraging spiritual engagement and hobbies as part of daily life. Rehab and recovery programs include physical, occupational, and speech therapy with licensed specialists, and every resident has a care plan built with nurses, therapists, and dietitians involved, whether they stay long-term or just for a short time after a hospital visit. Language support is available, and while the main language is English, some staff speak other languages, which can help residents feel more comfortable.

The community hasn't released specific office hours and is not currently accepting new patients as of the latest information, and although more detailed data can sometimes be hard to find online due to access errors, the dedication to health and help for older adults living at Columbine Commons shows in their commitment to offering care, safety, good food, and activities, all in a home-like place made to make life a little easier, a little safer, and a little more enjoyable for every resident.
