    Nice community, but care issues

I'm glad my mom lived at Bartram Lakes/Amazing Grace for four years - the small, park-like community, personalized memory care and lively activities made it feel like home and staff often went above and beyond. It's clean, bright, family-oriented and great for fairly active residents who want social programming and peace of mind. That said, on-site medical care is limited (LPN level), and we saw spotty communication, staff turnover, food and billing issues, transportation problems and occasional serious lapses in response/compassion. I'd recommend a tour, but only if you confirm nursing coverage, emergency response times and billing practices beforehand.

About Bartram Lakes Assisted Living Facility - Brooks Rehabilitation

Bartram Lakes Assisted Living Facility - Brooks Rehabilitation sits on a quiet 115-acre campus in Bartram Park, Florida, and offers care for seniors who want help with daily living but still want to keep some independence. The facility sits within Brooks Rehabilitation, which has served the community since 1970 and is known for its advanced rehabilitation, therapy, and recovery services. With 61 well-kept apartments, including studios, one-bedrooms, and two-bedrooms, residents have options that feel more like home. The Green House® Residences provide a different layout for seniors who want a smaller, more home-like setting, and there are services for people with developmental disabilities, too. The grounds have a fishing pond, walking trails, a solarium with wall-to-wall windows, and other nice spots to enjoy the outdoors, and inside there are comfortable living rooms, an activities center, a fitness center, and on-site dining that faces scenic pond views.

The staff at Bartram Lakes Assisted Living Facility - Brooks Rehabilitation helps with daily needs, and nursing staff are available at all times. The care team helps residents with daily activities and offers specialized services for memory care, short-term respite stays, and rehabilitation. Brooks Rehabilitation provides extra services such as physical therapy, occupational therapy, and speech-language pathology, as well as advanced care for issues like brain injury, spinal cord injury, stroke, orthopedics, and pain management. Residents who need help going home after a hospital stay can get support here. The facility's focus on community health means there's careful attention to rehabilitation that helps people regain strengths and abilities, which can make a big difference for those with health challenges.

Bartram Lakes Assisted Living Facility - Brooks Rehabilitation also puts a lot of thought into keeping staff supported. Management encourages good communication and teamwork, and most employees say they feel respected on the job, even though all staff report that stress can come up. There are efforts to fill shifts well and give enough resources, which aims to ease burnout and keep care at a high standard. The facility offers a part-time activity assistant position that's open Fridays and Saturdays, and the job includes tasks like setting up group activities, tracking supplies, and checking what new residents like to do. Staff need a high school diploma or GED and at least two years' experience for these roles, which focus on creating positive experiences for residents every day.

Along with help and care, the facility has a busy activities program, keeping people engaged and social. There are wellness and rehabilitation programs, group and one-on-one activities, and chances for both quiet time and social fun, whether that's getting hair or nails done on-site, walking the trails, or joining fitness classes. Safety and independence are respected here, so most services aim to help people do as much for themselves as possible. The place holds a Silver National Quality Award from 2018, which shows attention to quality, but daily life is pretty simple and down to earth, focusing more on comfort than flash. Meals come from a restaurant-style kitchen, where residents eat together and look out on the pond. Insurance may cover some types of care and therapy. Altogether, the setting and services at Bartram Lakes Assisted Living Facility - Brooks Rehabilitation reflect a goal to help people age with dignity, respect, and chances for growth.
