Owyhee Health & Rehabilitation Center sits in Homedale, Idaho, and has a 49-bed skilled nursing facility licensed by the State of Idaho, so you'll find they take care of a smaller group, which lets them focus on each resident, and they have a strong focus on patient care services, using both high-tech equipment and hands-on care to create therapy and care plans that fit the needs and wishes of each person, and they offer many types of care like independent living, assisted living, memory care, long-term skilled nursing, home health (Medicare-certified), non-medical home care, and even adult day services so people have a lot of choices depending on what fits their situation. You'll find a range of therapies here, including occupational, physical, speech therapy, respiratory therapy, and specialized support like lymphedema care, wound care, pain management, stroke recovery, and even palliative and hospice care, along with a full-time restorative assistant who makes sure daily programs set up by physical therapists get carried out. They work as a team with nurses, clinicians, and therapists, plus the person and their family, to make treatment plans that help each resident stay as healthy and comfortable as possible, and they even have a variety of daily activities and outings for folks to stay engaged, along with nutritional counseling and recreational therapy, so quality of life doesn't get overlooked. Owyhee Health & Rehabilitation Center has a state-of-the-art facility and a leadership team, and provides accessibility options for people with vision or language needs, including help in English, Spanish, Chinese, Vietnamese, and other languages. The center is part of Trinity Health Plan, accepts Medicare and Medicaid, but doesn't have a Continuing Care Retirement Community setup or resident/family council. It's earned a consistent 5-star overall rating for quality of care on Medicare.gov, and has held an A+ grade in inspections for several years, putting it in the top ten percent of nursing homes across the country, so people looking for strong health management and rehabilitation often look here when they need anything from short or long-term stays, respite care or even home-based help. It's a for-profit, individual facility with a history of highly rated patient care, and while it doesn't have every possible amenity mentioned, it does offer online payments, accessibility tools, therapy resource links, and keeps people updated through its website with news, photos, and a way for job seekers to see what's open for nurses, therapists, clinicians, and aides who want to join their team.

Caring staff, great therapy, monitor
I experienced deeply caring, responsive staff, an incredible admin team, excellent therapists, a clean welcoming facility and therapy that genuinely helped regain independence. However, I also witnessed serious lapses - nurse miscommunication, poor responsiveness, missing records, failures in oxygen/fall precautions, soiled care, a hospital transfer after unresponsiveness and even a death. Bottom line: wonderful, family-like people and top therapy, but monitor care closely - I recommend with strong caveats.
