    Good therapy, but alarming neglect

I have mixed feelings. Many therapists and aides were warm, caring and went above and beyond - good rehab, activities and a family-like feel - but I also saw alarming issues: heavy sedation/medication, missed basic care, hostile staff, loud paging/TVs, hot/cramped rooms and pest problems. I would not place a loved one here until management fixes staffing, medication practices, cleanliness and noise.

    About Oak Creek Rehabilitation

Oak Creek Rehabilitation sits as a facility that offers nursing care and a variety of rehabilitation services like physical, occupational, and speech therapy, and you'll find the staff helps with medication management, wound care, IV therapy, diabetic support, and even specialized cardiac care for conditions such as CHF, COPD, and recovery after surgeries like knee or hip replacements, and then there's help for memory care, behavioral care needs, adult day services, home health care, respite, and hospice care. The building holds 57 certified beds and most days about 39 residents stay there, and they've got both private and semi-private rooms with standard amenities along with communal areas for dining and social activities, and the meals aim to be nutritious and pleasant. The nursing staff provides 24/7 care under physician direction, but the nursing hours per resident per day are 3.01, which falls below the state average of 4.1, so while staff works hard, there may be less support at times than elsewhere, and nurse turnover is higher than average, with a rate of 72.9% compared to the 46.4% state rate, which might mean you see new faces more often than not. Inspections have found 10 deficiencies, including one related to infections, and there have been documented failures concerning resident protection from abuse, neglect, and actual harm, plus a couple of issues around not reporting suspected abuse quickly, so while they try to serve people's health needs, these are important things families might want to know about. The community accepts Medicare, Medicaid, and private insurance, and while their scores for staffing and inspection reports are lower than many other places, their quality measures rate much above average, and the overall CMS rating sits just slightly below average-not the highest, but not the lowest either. You'll find therapists on site who work to help folks recover and get back home when possible, and Oak Creek Rehabilitation organizes community activities, offers both short and long-term care, and keeps an eye on the basics like meals and room options, providing services in a home-like setting without any unnecessary frills.
