    Excellent care, poor management, pricey

I had a mixed experience. The rehab, daily physical/cognitive activities, memory care, tidy rooms, gardens and welcoming floor staff (Ashley and Patty were exceptional) made my loved one feel cared for, but management and communication were poor. Ongoing construction, an aging building, occasional neglect (lost items, missed meds, call lights out of reach), hallway odors and high costs are serious drawbacks.

    About Fireside House of Centralia

Fireside House of Centralia is a medium-sized nursing facility located in Centralia, Illinois, and offers both short-term rehabilitation and long-term care for its residents. The facility accommodates up to 98 residents and operates under for-profit, individual ownership. It is not part of a continuing care retirement community but participates in both Medicare and Medicaid programs, making it accessible for individuals relying on these forms of payment for their care needs.

The care provided at Fireside House of Centralia focuses on serving individuals who require daily medical assistance, recuperation from hospital stays, and non-medical support such as help with dressing, eating, and using the bathroom. The care home's short-term rehabilitation services are designed for those recovering from hospitalizations due to events such as strokes, heart attacks, infections, or injuries. Approximately 51.2% of residents who undertake short-term rehabilitation are able to return home after their discharge, reflecting the facility’s efforts in promoting recovery and independence. Nurse staffing at the facility averages about 3 hours and 20 minutes per resident per day, which is consistent during both the weekdays and weekends, providing stability in the level of care offered throughout the week.

Long-term care residents at Fireside House of Centralia benefit from a supportive environment aimed at maintaining their ability to perform everyday activities. A significant proportion of residents are able to continue moving, eating, and managing personal care without full assistance. Preventive health measures, such as annual flu vaccinations, are prioritized, with nearly all residents receiving their seasonal immunizations. The rate of major falls and infections requiring hospitalization are tracked to monitor and continually improve safety and resident health.

Fireside House of Centralia is subject to periodic state inspections evaluating health, fire, and safety standards, including food preparation and resident protection protocols. These inspections assess the quality of the environment provided and identify areas for improvement, with a focus on minimizing hazards and ensuring resident safety. Staffing levels during weekends are maintained at levels above average, which supports consistent resident care regardless of the day of the week. Emergency room visit rates and infection management are closely monitored as part of the overall commitment to resident health.

Overall, Fireside House of Centralia provides a balanced approach to rehabilitation and long-term care, aiming to help residents regain or maintain independence while ensuring a safe, supportive, and medically attentive environment. The facility’s structure and processes are tailored to address both the immediate and ongoing care needs of individuals requiring skilled nursing services.
