    Neglect sanitation theft poor communication

I had a mixed but mostly alarming experience. My loved one was left two days in bed with a dirty diaper and blisters, call lights and pamper changes were slow, staff seemed short-staffed/short-tempered with high turnover, and I noticed sanitation and theft concerns (roaches in the elevator, missing meals/groceries). That said, receptionists, a few CNAs, the nurse and physical therapist were friendly, and the small building felt clean and had therapy available. Overall, poor communication and neglect outweighed the positives - I would not trust this place for serious care.

About Irving Park Living and Rehab Center

Irving Park Living and Rehab Center on 4340 N. Keystone Ave. in Chicago provides skilled nursing services for people who need daily help, so residents get care like medication management, bathing, dressing, transfers, and steady nurse supervision, and there's a 24-hour call system always ready. This center is good for folks who need help with daily living or have serious medical needs, including those who can't walk, since the building handles non-ambulatory care and has dedicated aides who pay attention to each person's needs. The place gives memory care for people with Alzheimer's or dementia, and offers both short-term rehab for people getting back on their feet after the hospital and long-term care for the frail who depend on nursing support. Residents can get hospice care, physical therapy, occupational therapy, and medical help from skilled staff, who give about 4.29 nursing hours per person daily, which is more than most places, although the nurse turnover rate is higher than the state average. The facility offers many comforts, like private bathrooms, cable TV, furnished rooms, kitchenettes, air conditioning, phones, and free high-speed WiFi, plus emergency alerts and help with insurance, Medicaid, VA benefits, managed care plans, or private pay. The kitchen is modern and staff serve 1 or 2 nutritious meals each day, with menu options for dietary needs based on a registered dietician, and community dining means residents can eat together, although reports show issues with how food's sourced and served since staff haven't always met nutrition standards. Activities go on every week, with bingo, movie nights, outings, game nights, arts and crafts, gardening, a book club, musical group, spa/wellness room, birthday parties, and offsite trips, and there's a library, gym, outdoor patios, garden space, story time, and on-site religious services. Housekeeping, laundry, haircuts, room service, and transportation help people focus on living, and the staff can help with Medicaid applications, too, while pets are allowed for those who have them. The building is easy to get to with good parking and public transit nearby. Some things aren't perfect-state inspection reports list 63 total deficiencies, including about infection control and supervision, with 15 cited in January 2025 like five about infection issues and some about accident hazards, so supervision and safety have had documented problems that caused harm but nothing labeled as immediate jeopardy. The ownership is split between Ephraim Braunstein and Aharon Diena, and there are 117 certified beds with about 77 people living there each day and 40 beds open in June 2025, so there's room for new residents. People interested in both short-term rehab or long-term living, who need lots of nursing help or specific rehab, can find services here, but it's important to know about those inspection findings if safety or infection control is a primary concern, as the center is licensed, accepts Medicare and Medicaid, and can handle a wide range of medical and personal needs for seniors.
