Sheridan Village Nursing and Rehabilitation Center sits right along Lake Michigan in Chicago, and while it doesn't look fancy from the outside, the building holds 191 beds for people needing skilled nursing, psychiatric care, and specialized programs, with rooms facing the lake for a nice view when the weather's clear, and they try to give each person personal and goal-based care, whether for recovery or long-term help. The place is open all day, every day, with an experienced staff offering a wide range of services, such as skilled nursing, short-term and long-term rehab, physical, speech, and occupational therapy, plus behavioral care like psychological testing, and even special programs called Adapt and Moving On to help people settle into the community, and they say their team keeps a close eye on both medical and mental progress, making sure folks get what they need as they recover or adjust. They offer memory care and palliative care for those who need extra help, along with medical services like IV antibiotic therapy, wound care, diabetes care, pain management, colostomy and catheter help, dialysis transportation, and more, and there's a big focus on therapies that go beyond the basics, including recreational therapy, nutrition counseling, and even a monthly activity list to keep spirits up. Staff offer social services to help residents plan for going home when they're ready, support with daily life skills, and help connecting with the wider community, while daily living gets some comfort with small touches like an in-house salon offering free haircuts, community councils for both residents and family, lakefront sitting areas, and transportation for appointments. The facility accepts Medicare and Medicaid, provides both personal and medication care, and hosts tours for new residents and families curious about life there; yet, despite the many services, it does face its own challenges, as shown in recent CMS inspection reports where inspectors found 46 total deficiencies, four of them involving infection control, with some citations for not protecting residents from infection, taking too long to report concerns of abuse or neglect, and not responding properly to all reported violations. Ownership is split between individual owners like Yaacov Mashiach, Rita Lipshitz, Rhonda Mashiach, Yechiel Mashiach, and a company called Atied Associates, and while the nurse turnover rate is lower than the state average at 31.5%, the nurse staffing hours per resident per day fall below the average, with 2.33 hours compared to the state's 3.4, and while folks have said staff are compassionate and amenities decent, average online reviews tend to be mixed, with the last check showing a 2.8 out of 5. The place is known for focusing on rehab and recovery through a proactive approach, but it's had both B-minus and C grades in inspections over the years-not without issues, but with some above-average performance in other areas.

Neglectful filthy abusive uncaring facility
I placed my loved one here and it was awful. Staff were mostly uncaring, money-driven, and at times abusive; residents were neglected - frequent falls, ER trips, bruises, broken nails, unwashed hair for months, and overmedication. The place was filthy and poorly maintained (elevator repairs needed), administration rude and uncommunicative, and safety felt ignored; only a few employees tried to help. I would not recommend this facility.
