    Pleasant staff but safety concerns

    I had a mostly pleasant stay - the staff, therapists and social workers were kind, hardworking and polite, the dining and food were good, and rooms and common areas were extremely clean with a pleasant smell. That said, I witnessed troubling safety and care lapses (medication/dietary mistakes, shortages of basic supplies, communication problems) and a nighttime oxygen emergency that required ambulance transfer, which left me very concerned.
