Forest City Rehab sits at 321 Arnold Avenue in Rockford, Illinois, and has room for up to 213 people at a time, so you'll see a variety of folks coming and going, some staying only a short while for respite care and others staying longer, and since they have 24-hour skilled nursing care all week, there's always nurses around day and night, and if there's ever a crisis in the middle of the night, they've got crisis intervention, assessment, and admissions available right away, which is good peace of mind, especially for families who worry about sudden changes. They handle Alzheimer's and dementia care with staff trained for memory support and safety, and they keep plenty of different therapies available, making sure therapy plans are matched to each person, whether someone needs help after surgery or because of a long illness. The facility also runs hospice care for those who need comfort near the end of life, with a focus on dignity, and offers respite care so caregivers at home can take a break now and then, knowing their loved one has the supervision needed in a safe place. Their rehab center covers a good range of rehabilitation programs, a variety of therapies, and discharge planning, which helps people move from the hospital or another setting back home or to the next stage. They keep social care services on hand, plus recreational activities for the residents, and there's always nutritional support, so meals are handled, which matters if someone can't cook or shop by themselves anymore. Forest City Rehab is licensed by the State of Illinois, is certified by the Joint Commission, and takes Medicare, Medicaid, and many private insurance plans, which takes some worry off when it comes to paying for care. The facility's linked in with the Social Care Network, and they offer onsite services, including disability parking for easier visits. They keep their doors open for visitors every day from 8 in the morning until 8:30 at night, and the office handles business weekdays from 8 to 4:30, making it simple for folks to call, email, or drop by during those hours if needed. Filthy facility neglectful and unsafe
I would not recommend this facility. I experienced filthy, unsafe conditions - infestations (bed bugs, roaches, flies, maggots), urine-soaked wheelchairs, lost/stolen items - and neglectful care (missed meds, long call-light delays, bedpan left for hours, rushed/failed discharges that ended in hospital). Management seemed aware but unresponsive, and I felt abandoned and afraid for my loved one's safety. A few staff (Angie, Kendra, Stacy and some nurses/CNAs) were caring and helpful, but the negatives far outweighed the positives - look elsewhere.
