    Caring rehab but safety concerns

I found a small, home-like place with genuinely caring, knowledgeable staff - my family saw real progress in post-stroke rehab and appreciated open communication and strong family-staff relationships. However, I also witnessed staffing shortages, turnover, medication/care lapses that led to falls, unclear explanations and trust issues after management changes. Overall it can be wonderful, but I'd insist on asking direct questions about staffing, safety and reporting before placing a loved one.

    About Hickory Creek at Franklin

Hickory Creek at Franklin, located in Franklin, Indiana, has served seniors in the area since 1970 and offers a range of care options such as Independent Living, Assisted Living, Memory Care, Home Care, Adult Day Services, Long-Term Care, Home Health Care, and Hospice Care, so it covers many common needs as people age, and the staff, who work to become an extension of residents' families, include experts in senior care who help find answers and bring peace of mind for residents and loved ones. The facility isn't a continuing care retirement community, but it does provide skilled nursing services, hospice, short-term rehabilitation, and respite care, with programs designed for individuals recovering after surgeries or illness, as well as those who need extra help with daily tasks due to age or health limits, and for families who need a break from caregiving. The community uses a team approach and supports residents who may need help with everyday activities, deal with pressure sores, or have issues like urinary tract infections, so licensed nurses, certified nurse aides, doctors, therapists, and other specialists create personal care plans after reviewing each person's needs. Those who stay long-term usually get yearly influenza shots (93%) and pneumococcal vaccinations (86%), helping reduce risks for common illnesses, and they benefit from physical, occupational, and speech therapies aimed at maintaining as much independence as possible.

Staff hours per resident per day are 1.11, which includes 0.48 hours with registered nurses, 0.63 with licensed practical nurses, and 1.64 hours with certified nurse aides, so there's support throughout the day and night since the facility's open 24 hours daily. The building's fully sprinklered for fire safety and supports resident and family councils, creating spaces for sharing ideas or concerns. Hickory Creek at Franklin has 38 certified beds, with 37 currently filled, and participates in Medicare and Medicaid programs, helping cover a range of care costs with assistance available from Medicaid specialists. The community welcomes older adults who are active as well as those needing more support, offering both a home-like, cheerful setting and secure environments, especially for memory care. The facility's part of American Senior Communities, operates as a non-profit corporation, and is involved in multi-nursing home ownership, so it has experience managing different needs and care programs. Health inspections and nurse staffing data are available, and while the state's reports have noted issues such as mistreatment, neglect, theft, food safety, resident assessments, and quality care deficiencies, these reports give a clearer picture of areas where the community works to improve and meet standards. The community has a website at asccare.com/community/hickory-creek-franklin for more information about care offerings, programs, and daily life at this location.
