    Caring staff, clean facility, cautious

I had a mixed but overall positive experience. I found the building beautiful, spotless, and ideal for rehab/long-term care - staff across roles (nurses, PT/OT, aides, drivers, admin, custodians) were caring, attentive, coordinated with hospitals and hospice, and helped my loved one make real progress. That said, meals and evening/front-desk phone service were poor, a few staff were rude, and I witnessed occasional clinical/communication lapses (missed checks, medication/notification issues), so I recommend the facility but advise staying vigilant about meals, evening coverage, and family communication.

About Creekside Health and Rehabilitation Center

Creekside Health and Rehabilitation Center offers a wide range of care for seniors, including independent living, assisted living, memory care, and skilled nursing, so people with different needs can find support, and you might also see services like home health care, non-medical home care, and adult day programs which is helpful for some families, plus the center takes Medicare, Medicaid, and long-term care insurance, so paying for care is a little easier for some folks who need it. The private rooms run about $6,000 to $10,000 a month and semi-private rooms cost between $5,000 and $7,000, and you don't have to worry about a buy-in fee since it's a rental community, so you avoid a big, long-term financial deal up front. There's air conditioning throughout, parking for guests and residents, and restrooms are easy to find, as well as wheelchair accessibility built in, and safety features like sprinklers, handicap-accessible designs, and washers and dryers for residents are available, which can make day-to-day life much easier. People using Creekside will see services like housekeeping, kitchens or kitchenettes in their rooms, cable TV, regular maintenance, and a salon and barbershop onsite, so daily personal needs are taken care of, too, and if you like having games, social activities, fitness center, or programs for health and learning, you'll find those along with crafts, Wi-Fi, guest parking, and a dining room. When it comes to personal care, there's help with dressing, grooming, bathing, and toileting, and personal care assistants are there for walking and wheelchair assistance, plus laundry and dry cleaning is also available, so you're not left worrying about small, daily tasks. You also have regular meals provided, and people get medical care such as wound care, medication support, nurses, occupational therapy, and podiatry, with extra services for things like amputee rehabilitation, so folks with high care needs have support, and those wanting short-term rehab after a hospital stay or long-term skilled nursing can find the right fit. There's transportation service for those who need to get to appointments or activities around town, and people have choices for staying private or sharing a room depending on the budget and preference. The place offers hospice care for those in need, and there's around-the-clock nursing support-which means someone is always there if help is needed, and they say they focus on restoring health and keeping life active and comfortable. Creekside's CMS rating is slightly below average, staffing is average, quality measures are above average, but health inspections are a little below average, so while it's not perfect, many people find the range of services and care options fit what they're looking for, and the atmosphere tries to be supportive and comfortable for everyone; plus, with four pictures of the facility available, people can get a look at what life is like before they decide.
