    Excellent food, inconsistent care, unsafe

I chose this nearby, long-tenured facility for the excellent food, fun activities and some genuinely caring staff, and parts of care were excellent. However, the building is dated and sometimes smelly, staffing is often inadequate or unprofessional (rude nurses, lost clothes, privacy concerns), care was inconsistent with poor pain control, limited/no on-site assisted living or PT, and a premature discharge even required a hospital visit - so I wouldn't recommend it without checking staffing, PT availability, and safety.

About Northwest Manor Health Care Center

Northwest Manor Health Care Center has 126 certified beds and accepts both Medicaid and Medicare. The center, affiliated with Adams County Memorial Hospital, has an "Average" overall rating from CMS, though it scores above average in quality measures, and people can find detailed information and inspection reports through its profile. The center offers a wide range of services including Independent Living, Assisted Living, Memory Care, Home Care, Adult Day Services, Long-Term Care and Skilled Nursing, Home Health Care, and Hospice Care, and it has a Transitional Care Unit for patients needing short-term rehab after a hospital stay or for those who need complex care and extended recovery time. There's skilled rehabilitation, orthopedic rehab, and rehabilitation services seven days a week, with advanced therapy equipment and a team that's used to working with aging adults who have medical complexities.

There's also on-site dialysis through DaVita and a Geriatrician available throughout the week, private fully furnished suites, TV, cable, and phones in every room, which makes things more comfortable for residents, and the social environment stands out because activities help people stay mentally, emotionally, and physically engaged. Meals are prepared by chefs and meal planners who focus on nutrition, and memory care is designed to help prevent wandering and reduce confusion for those with Alzheimer's and other forms of dementia. Staff at Northwest Manor Health Care Center are generally known for being joyful, kind, and helpful, which helps make things feel welcoming for the residents. The center sees a nurse turnover rate of 32%, which is lower than the state average of 48.5%, and it offers more nursing hours per resident each day than average, logged at about 4.18 hours compared to the state's 3.7.

The place has had 20 inspection deficiencies in recent reports, which includes issues like not always keeping resident information safe, not labeling or storing medicines properly, and sometimes not getting food from approved sources, with no direct harm found from these but noted risks. There have also been cited deficiencies in nutrition, pharmacy services, and care planning, which are important for families to know. The center tries to keep things home-like and cozy, encourages families to stay involved in care plans, and has staff available for therapy seven days a week, especially for those in need of physical and medical rehabilitation after illness or surgery. While there's no detailed list of amenities or special program names, rooms come with basic comforts, and the environment is generally friendly, with care designed to fit the needs of seniors who need support short-term or for the long haul.
