    Good care but unsafe conditions

I'm grateful the staff gave loving, individualized, proactive care and my loved one did well, but persistent black mold, awful food, and safety concerns about other residents mean I wouldn't send someone there.

    About Brookside Haven Care Center

Brookside Haven Care Center gives care to seniors in a large, comfortable community where staff members help with daily needs like bathing, getting dressed, using the bathroom, and taking medicine, and there are people on hand around the clock for extra support, so folks don't have to worry about being alone, plus there are locked and alarmed areas to prevent wandering for those who need memory support. You'll find special areas for activities such as a book room, spa, steam room, fitness center, hair salon, and garden spaces, and they run community night events, movie programs, and art classes that help people stay social and entertained, while daily schedules include music programs, board games, pet therapy, creative activities, walking trails, outside trips, and even a gym for regular exercise fit for different abilities, so whether someone's up for a lot or just a little, there's something scheduled every single day. Meals get prepared fresh by the kitchen staff three times a day with extra snacks in between, and the cooks make special meals for folks with needs like diabetes or high blood pressure, and there's even a traveling stylist who comes in for haircuts. Laundry, housekeeping, and dry cleaning get taken care of too, and staff also help coordinate and provide rides to doctor's appointments, errands, or religious services, since transportation is available as part of the care.

Rooms have washers and dryers, cable TV, safety systems, and some have kitchenettes, while the community itself brings wifi, guest parking, activity rooms, dining areas, and relaxing porches where people can gather or watch the day go by. Insurance options include long-term care coverage, and some families use it to help pay for care. Brookside Haven serves elders needing different levels like assisted living, independent living, memory care for conditions such as Alzheimer's and Parkinson's, skilled nursing, rehabilitation, respite care, and long-term care, with specialized programs for both physical therapy and wellness led by licensed experts and therapists who know what they're doing.

Staff offer care plans tailored to each person's health and psychological needs, with regular health checks, nurse support, medication help, and personal care assistants. People speak different languages here, so there are options for folks who aren't comfortable using English. If you want to see the place, staff can show you around and answer questions, and potential residents are welcome to meet people and join a tour to get a feel for daily life.

The community also provides a secure environment with systems in place to keep everyone safe and monitored, and there's a resident council that meets, though there's no family council present. While Brookside Haven has many services, it's important to note that it's had an abuse flag recently, and its quality grades for long-term care have ranged from D to B, which shows mixed results in some areas, and its nurse grades and state inspections have hovered around B-minus, but its short-term rehab services rate a B-. Members of Nationwide Healthcare Services run the facility, and overall, the place mixes regular care, rehabilitation, art and music programs, health services, and a homelike setting for elders who need different types of help and treatment over time. Sometimes information on Brookside Haven can't be found online due to server problems, so it's best to talk to staff directly or visit in person if you want up-to-date details.
