    Excellent frontline care, management inconsistent

I chose this facility for my mom and have mixed feelings. The CNAs and therapy team were incredible - Simone, Alex, Nicole (and Amanda) went above and beyond, were kind, attentive and helped my mom improve; emotional support and rehab were excellent. Unfortunately management and some staff are inconsistent: long call-light waits (~10 minutes), poor responsiveness, rude attitudes at times, spotty communication with doctors, worries about overmedication and PHI privacy, and they won't take dialysis patients. I also observed run-down rooms, cleanliness and infection-control concerns and have heard reports of serious safety incidents. Bottom line: great frontline staff, but verify the unit, monitor care closely, and be prepared to push management on issues.

About Golden LivingCenter - Fountainview Place

Golden LivingCenter - Fountainview Place in Portage, Indiana, has 186 beds and offers both assisted living and skilled nursing care in a large community setting. People find one-bedroom units, studios, and semi-private quarters, all with air conditioning and private bathrooms, some furnished with kitchenettes, cable TV, and Wi-Fi. Staff stay awake and on duty 24 hours a day, with emergency response services for resident safety and a secure memory care unit for those living with memory loss, including extra supervision and tailored daily support, so families know their loved ones are monitored and protected.

There are walking trails, gardens, and outdoor spaces, movie theater rooms, a cafe, activity and arts rooms, and a fitness room, plus a hot tub, library, game area, and regular chapel service. The community welcomes small pets and sets up planned daily activities-things like movie nights, craft sessions, fitness classes, music therapy, board games, and outings for shopping or religious services-so everyone can keep busy and have company when they want. Residents can enjoy three homemade meals a day in a full-service dining room, with nutrition needs considered for special health conditions, as well as laundry and housekeeping help for comfort and convenience.

Trained nursing staff help with daily needs, including bathing, getting dressed, transfers, eating, and mobility, and they follow care plans for each person. The staff provides care for those with more complex needs too, including injections, respiratory care, colostomy or tracheostomy care, and foot care, and supports people with prostheses. There's a memory care program with dedicated activities and extra safety for people with dementia or wandering risk, including Wanderguard monitoring. The center supports rehabilitation after illness or surgery, offers intermediate and long-term care, short-term stays, and respite services for families who need a break.

Laundry services are part of the amenities, and special clothing for mobility or dressing needs is available, like wheelchair pants, open-back tops, and adaptive undergarments, with a range of sizes and easy-dress choices. A barber and beauty shop operates on-site, and medication management, help with appointments, and transportation services are offered as needed. Families and residents have councils for feedback and community events, and there are social gatherings to support resident and family connections. The premises are fully sprinklered and kept free from hazards, while all personal money held by the facility is properly managed. The place operates as a for-profit corporation, not in a hospital, and supports both Medicaid and Medicare. It's licensed in Indiana for both nursing home and skilled nursing care, and while the beds are nearly full, there's still a steady turnover from short-term rehab stays. Everything's set up to offer a safe, home-like environment with as much independence as possible for its residents.
