    Strong rehab, disappointing long-term care

I had a mixed experience. The nursing and physical-therapy teams were wonderful - attentive, compassionate staff and management often went above and beyond and gave regular updates. But the facility felt old and understaffed: rooms and bedding sometimes weren't cleaned, odors/bugs were reported, meals were poor, meds and assistance were delayed, and I saw inconsistent professionalism, safety and communication problems. I'd trust them for short-term rehab/PT; I would hesitate to place someone there for long-term care.

About West Bend Nursing and Rehabilitation

West Bend Nursing and Rehabilitation is a care facility in South Bend, Indiana, offering nursing and rehabilitation services for seniors and folks who need extra support after medical conditions like a stroke or surgery, and they've got 157 certified beds with an average of about 56 residents each day, which leaves some room open for new folks if they're looking, and you'll see options for semi-private rooms ranging from $6,000 to $8,000 per month and private rooms that run between $7,000 and $9,500 per month. The building comes with basic comforts like air conditioning, wheel-chair accessibility, high-speed WiFi, cable TV, and housekeeping, plus shared areas like a dining room, fitness center, game room, and outdoor patios or gardens where residents can gather or relax. Staff runs the place 24 hours a day, working to keep residents feeling safe and looked after, and they offer everything from laundry to personal care like dressing, grooming, and help with medication, as well as services like occupational therapy, wound care, and even hospice if someone needs it. Residents can get social with activities such as arts and crafts, a musical group, story time, game nights, and in-house religious services-they also have meal service with hot food, room service, washers and dryers for everyone, and a barbershop or salon when someone wants a trim. For health and recovery, they do have a "Road to Recovery" program where doctors, nurses, and therapists set up plans with residents within the first three days and provide therapy for mobility, pain management, and more; they also offer outpatient rehab for those who don't live there full time. The staff aims to treat residents like family, keeping families updated and stepping up during emergencies, though it's worth knowing they do face staffing challenges, and nurse turnover stands higher than average at 63.1%, but they still manage to provide more nurse hours per resident per day compared to the state average. The building's management comes from American Senior Communities LLC, which has run things since 2011, and the facility is known for being a bit older in style, with some describing it as outdated, but they keep things clean and comfortable with features like community laundry and on-site parking for guests and residents. Residents get meals and snacks, cable, internet wired in rooms, and property features designed for safety like handrails, and there's an emphasis on making sure folks aren't at risk of falling. It's important to mention that inspections found 28 deficiencies, including areas like resident care planning, resident rights regarding notification on transfers, and issues with food procurement and storage, and there have been infection-related deficiencies too, so checking those inspection records is a good idea. The place accepts long-term care insurance and is set up to help with everything from daily personal care to medical and wellness programs, and the staff, while sometimes stretched, have shown dedication and compassion, doing their best to make residents comfortable and involved in daily community life.
