    Excellent rehab, negligent long-term care

    My experience was deeply mixed. The rehab team, Cindy and several therapists, were excellent - private therapy rooms, daily PT/OT, engaging activities, friendly admissions, and some very caring CNAs made recovery possible. But nursing and administration were inconsistent and often negligent: missed appointments and meds (including insulin mistakes causing hypoglycemia), poor bathing and hygiene, long waits, residents left in urine/feces, bed-sores, UTIs and infections, rude or inattentive nurses, chaotic discharge planning, and spotty communication. The building and care feel understaffed and money-driven at times despite pleasant areas, clean rooms, and warm activity staff. I'd recommend it for short-term rehab only; I would not trust it for long-term, high-acuity care.

    Heritage Healthcare sits in Indiana, where Life Care Centers of America owns and manages it as a government county facility, and Lafayette Medical Investors Limited Partnership has had a hand in running things since September 2018, with other organizations involved after that. The place can care for up to 127 residents in certified beds, though the census usually counts about 78.     Residents have access to many levels of care, including 24-hour skilled nursing, long-term care, memory care for those with dementia, assisted living that helps with daily tasks, and independent living for active seniors who want fewer chores. There's also home care available if someone wishes to stay at home but needs a bit of help. Those needing recovery after a hospital stay can move into post-hospitalization suites, use inpatient or outpatient rehab, and rely on therapies like physical, occupational, speech, anodyne, intravenous, oxygen, and suction treatments. Social services, case management, medication oversight, wound and diabetes care, and fall and infection prevention programs are all part of the support, as are specific services for discharge planning and community re-entry.

    Housekeeping and laundry are provided, and folks can stay connected with family through flat-screen TVs, flexible visiting hours, and family or resident councils. The community puts effort into social, mental, and emotional activities, and has won awards for friendly staff and lively programming. Skilled nurses and on-site doctors make individualized care plans, using modern rehab equipment and therapies to help residents regain or maintain as much independence as possible. There's a record of community awards-like Best of Senior Living and All Star distinctions for kindness, support, and engagement. Heritage Healthcare, however, does have 25 deficiencies from inspection reports, including issues with infection control and one related to a resident's right to refuse transfers (F0560), and management is responsible for protecting residents from abuse, neglect, and exploitation. Heritage Healthcare offers a wide range of care options and tries to create a safe, caring, and welcoming environment, but it's also important to know about its inspection history and current efforts to address those specific areas needing improvement.
