Creston Specialty Care sits at 1001 Cottonwood Road in Creston, Iowa, and serves older adults who need skilled nursing or long-term care, and the people there seem to take their jobs seriously because they focus on care that fits each person, whether someone needs short-term rehabilitation or long-term support for chronic conditions, so you'll find staff on hand 24 hours a day looking after up to 74 residents, and the staff includes registered nurses, LPNs, CNAs, therapy teams, and medical directors who work together. People can get help with pain management, wound care, IV antibiotics, and medication plans, and there's physical, occupational, and speech therapy offered right onsite; folks living there can use meal programs and there are daily social and educational activities, which keep things from being dull, while specialized services like nutrition and dietary counseling and restorative care try to make life more comfortable for those with specific medical needs, like respiratory care or orthopedic rehab. The nursing home has a long history of providing compassionate care and doesn't boast about fancy awards but has gotten inspection grades like C and some A+ health inspections in recent years with no severe issues found, showing some attention to safe and steady practices. The place is non-profit, part of Care Initiatives, affiliated with networks like IA Health Care Association, and linked to Providence Health Plan and Trinity Health Plan, so they take Medicare and Medicaid, which matters for costs. People can get help whether it's short rehab after an injury, long-term care for ongoing illnesses, or even hospice and end-of-life support, and there are also respite care and both inpatient and outpatient rehab. The community tries to make folks feel comfortable and engaged with recreational therapy and active daily schedules, and while it does have multiple locations in Iowa, not all are taking new patients. There's a dedicated webpage on CareInitiatives.org with current details about the facility. There's no telehealth but the office is open Monday to Friday, 8 AM to 5 PM. The staff speaks several languages, though it doesn't say which ones, and they focus on making sure residents keep their dignity and as much independence as possible, aiming to support the residents and their families through every stage of care.

Caring staff but operational issues
I live at Creston and appreciate the caring leadership, compassionate team, positive culture, and staff dedication - nurses are even earning RN licenses and residents are clearly well taken care of (outstanding annual survey). That said, the food is often poor (mushy broccoli, shoe-leather pork chops, watered-down mac 'n' cheese), prescriptions seem overpriced, and response to calls for help can be slow - an operational/inspection review would help. Overall, amazing, super-nice staff - congratulations to Team Creston.
