Denison Health Care Center sits on Ridge Road in Denison, Iowa, and has room for about 46 people in private and semi-private suites, though the average daily census sits near twenty-five, and folks there get care from a staff clocking in just over three and a half nursing hours per person each day, right above the state average, which means there are enough hands to help but not so many that you'd get lost in the crowd. Arboreta Healthcare Consulting, LLC runs the place, and it's called Denison Care Center, with all the paperwork in order for Medicare and Medicaid. The center offers skilled nursing, rehabilitation after illness or surgery, long-term care, and hospice support for those at the end of life, with therapy like physical, occupational, and speech sessions available as well, plus wound care and tracheostomy management, and you'll find tube feeding, IV therapy, and even wound vac therapy on the menu, so they handle some pretty complex medical routines with a team approach that tries to build care plans around what each person needs most, although they've had some trouble in the past meeting rules about making timely and thorough care plans and other things like keeping up on nutrition standards, infection control, and making sure residents understand their payment responsibilities. Free Wi-Fi covers the building, cable TV and phone hookups are in the rooms, transportation and laundry get handled by staff, and there's a hair salon for a trim or curl. The place serves several meal options every day, lets residents practice their faith with weekly church services, and provides a secured environment for safety. They admit adults needing respite care too, so if family caregivers need a break they can get help, but don't offer telehealth visits, so seeing a doctor means someone comes by in person, usually on a regular schedule. Folks enjoy various activities, and the staff aims to provide a warm setting, but keep in mind - state inspectors have found about twelve deficiencies during recent reviews, mostly related to paperwork, nutrition service, infection spread, and communication about Medicare or Medicaid coverage, so it's wise to look over recent inspection reports, which you can find on their Medicare page, before making any decisions. The building has space to accommodate both short and long stays with a focus on helping each person improve as much as possible or stay comfortable.

Rude staff forced costly plan
I had a terrible experience. Staff were rude (they even hung up on my call), they forced a Medicare Advantage plan that left me with a $15,000 bill, and costs are outrageously high - I felt they took advantage of seniors. I don't trust them; I'd give half a star. Needs major improvement.
