Oskaloosa Care Center sits on Highway 432 in Oskaloosa, Iowa, and has 83 certified beds with about 79 residents on average each day, and folks who stay there can get care for memory problems with their Alzheimer's and dementia units, or they can get skilled nursing if they need more help, because the care center's got hospice care, wound care, psychiatric care, and licensed skilled nursing for long-term or temporary needs, and you'll find direct owners on record like Kenneth Carlson, Ronald Reitmeier, Barbara S. Donohue Revocable Trust, and Michael Theobald, but no information's listed about any indirect owners, and Tina Steffen leads as Administrator. Now the staffing ratio stands at 3.43 nurse hours per resident per day and there's a high nurse turnover rate of 70.7%, which means a lot of changes in who's caring for folks, and they've had deficiency reports related to resident safety and rights, but the place keeps services going for wound care, rehab, therapy-including speech, occupational, and physical therapy-and offers devices for folks who need help getting around. Staff is there around the clock, and residents get daily help with meals and housekeeping, with medication managed as needed, and there's a monthly activity calendar sent out and all sorts of activities and religious services available if anyone wants to join in, plus if loved ones have questions they can use the online form or get updates through email. The care center says it puts focus on quality of life and meeting community needs, with teams like Team OCC running daily departments in Nursing, Dietary, Housekeeping, and Laundry, while pushing for education and training for employees so they're able to deliver care with dignity, honesty, and respect for everyone, and all the rooms and shared spaces are kept clean and pleasant, never fancy but always functional, because the real goal's taking care of elders in the community and giving families a place to meet with staff and help their relatives stay healthy and involved.

Severe neglect causing mother's death
I liked the spotless, well-maintained building, homemade food, active therapy/activities, and the strong memory-care team - rooms are spacious and cleaned daily. Unfortunately the nursing/home care has gone downhill: severe staffing shortages, poor attitudes, bad communication, untreated infections and falls (catheter left >3 days), ambulance trips and ultimately my mom's death after what I believe was neglect. Administration seemed more concerned with money and was unprofessional/rude (director Tina). I cannot recommend this facility for families.
