    Happy resident excellent compassionate care

    I live here and couldn't be happier - the facility is clean, welcoming, and the nurses, CNAs, therapists and support staff treat residents like family. Therapy and rehab are excellent, activities and food are enjoyable, and staff communicate compassionately with families. I feel safe, well cared for, and would highly recommend this place.

    Minneapolis Health and Rehabilitation is a skilled nursing facility with 45 certified beds and an average of about 42 residents staying each day, and it's designed for seniors who need serious medical care, help after surgery, or support with daily activities like bathing, dressing, or getting around, and people who need short-term rehabilitation between a hospital stay and heading home often find all sorts of therapies and healthcare services here, along with meal plans made by chefs and meal planners who pay attention to special diets. The facility is affiliated with Recover Care Healthcare and owned by Midwest Snf Holdings LLC with several indirect owners. Residents get nursing care, rehabilitative therapy, memory care, palliative and hospice care, mental health services, and even respite care or help planning their discharge, and for extra support, there's a clinic with physicians onsite, a 24-hour call system, and all-day restaurant-style dining in a dining room where meal service is available throughout the day.

    The building itself has recently been remodeled to give a homelike feeling, and there are common areas, walking paths, an arts room, a barber and salon, plus communal areas for activities like devotional services, movie nights, and on- and offsite happenings, as well as personal room features like phones and furnishings, and laundry, housekeeping, and move-in help are all part of the package. There's a special focus on safety and accident prevention, but some past inspection reports show four documented deficiencies, including one labeled F0689 about keeping areas free from accident hazards and making sure residents have enough supervision, and while some of those inspections found issues that could lead to real dangers, others were marked as not causing actual harm but having the chance to do more than a little harm if left unchecked. Staffing levels sit at 3.53 nurse hours per resident per day, which is below the state average of 4.0, but nurse turnover is 45.5%-lower than the state average-so a mix of longtime and newer staff are caring for residents, some of whom have been with the center for many years and are nearing retirement.

    The community accepts Medicare and Medicaid, has council groups for residents and family to share concerns and help steer the care experience, and works hard to offer a safe setting through both skilled nursing and personal attention. For healthcare needs, seniors can get medication management, diabetic care, incontinence care, non-ambulatory care, along with support for mental health or wellness programs to help people keep their independence, whether they're staying for a short recovery or a longer time. Three certified beds are available for new residents in June 2025, and the facility is also part of a Continuing Care Retirement Community, which means people can change their level of care as their needs change. The center is equipped with therapy equipment and a warm, knowledgeable staff, and they say they want to give every resident care that's both loving and focused on the best outcomes, always keeping safety and quality of life at the top-even as they continue working to resolve noted quality issues and improve their operations for everyone involved.
