    Compassionate staff provide outstanding care

I couldn't be more grateful - the DON even reviewed records remotely to expedite our timely admission, and Nurse Megan and the rest of the team have been angels. Staff are compassionate, attentive, and truly treat residents like family in a clean, home-like building with outstanding nursing and therapy. I highly recommend this facility for the caring, communicative team that goes above and beyond.

About Yates Center Health and Rehab

Yates Center Health and Rehab sits in Yates Center, Kansas, serving as a skilled nursing and retirement community that's been around for over twenty years, offering a mix of care levels like independent living, assisted living, nursing home, rehabilitation, hospice, respite, and memory care in a place that feels comfortable. The center is Medicare and Medicaid certified and is owned by Infinia, a family company that also provides ambulance, home health, and hospice services, and it's part of Mission Health Communities. The facility has 45 rooms and about 31 residents as of now, and it's getting ready to rebuild with a new, modern building after a recent fire, with the new site expected by late 2026.

Residents get access to things that help make life easier, like housekeeping and laundry, 24-hour supervision with a call system, and maintenance and safety features for those with disabilities. The staff offers help with bathing, dressing, transferring, managing medicine, and daily activities. Meals get served restaurant style, with meal plans that can handle special diets. Both private and semi-private rooms come with nice touches-kitchenettes, cable TV, telephones, high-speed WiFi, and furniture already set up, which takes some worry off moving in. There are community dining spaces, social rooms, a library with plenty of books, outdoor spaces like gardens, a swimming pool, and a hair salon or barbershop, so there's always a spot to gather or spend some quiet time.

There's a strong focus on health and therapy at Yates Center. The staff provides in-house physical, occupational, and speech therapy in a gym with equipment for recovery. Treatment can match a lot of needs, from back strains, Parkinson's disease, arthritis, and tendonitis, to bigger medical challenges like cardiac, orthopedic, and pulmonary issues, wound care, sepsis, and complicated needs like dialysis care, trach care, and specialized IV therapies. Occupational therapy covers not just exercises but skills for daily living and even skills for managing the household. Special programs exist for people who need cardiac or pulmonary rehab, and services like psychiatry and counseling are here for residents and families. Nine complaints or deficiencies have been reported, though no fines or penalties have happened.

Life at Yates Center also means having a calendar full of activities, so there's less chance of feeling bored or alone. Residents can join arts and crafts, movie nights, spiritual and religious services, social events, education programs, and both on-site and off-site outings. Spots like a community dining hall, the swimming pool, library, salon, gardens, and common rooms make it easy for folks to come together or find peace and quiet. There's a mental wellness program and daily support from trained caregivers and nurses, who have been noticed for their good work, and senior care advocates help answer questions about living there or moving in.

Payment options are flexible, with Medicare, Medicaid, and insurance all accepted, which can help many families. Tours are available for anyone interested. The center supports all kinds of residents, even those with complex needs or disabilities. Transitional and short-term care, including orthopedic, wound, and IV therapy, plus respite and comfort care, is available for those needing extra time and help. The focus remains on health, dignity, and well-being, and the staff stick to the CARES principles: Character, Attitude, Respect, Excellence, and Service. The site's now in a rebuilding phase, but care and services continue, and the plan is for a new community facility to open in the near future.
