    Extremely happy, therapy dogs, improved

I'm extremely happy here - first-class therapy (therapy dogs!), caring, family-like staff, great activities and food, and with new management the facility is clean, well-maintained and focused on rehab. There were past issues, but I've seen real improvements and I highly recommend it.

About Beaver Dam Nursing & Rehab Center

Beaver Dam Nursing & Rehab Center sits at 1595 U.S. 231 in Beaver Dam, Kentucky, in a spot that's easy for families to find when they visit, and the place goes by the same name as the town, which helps, and you see, it's a for-profit facility managed by Douglas Cox since January 2015 and run as part of Providence Health Group, with services focused mostly on skilled nursing care and short-term rehabilitation, along with long-term care for people who need it. The building holds up to 58 certified beds and has about 55 residents staying there each day, so most of the beds are nearly full, and lately, they're not accepting new patients but do offer a phone number so people can check on future openings if needed. Staff at Beaver Dam Nursing & Rehab Center mostly speak English, and the nurse turnover rate sits at 37.3% with each resident getting about 4.40 nurse hours per day, and the team includes nurses, therapy specialists, dietary, and housekeeping workers, all working together to help the residents.

The place tries to keep a family-like atmosphere, and many people say the therapy and nursing teams offer good care, with therapy programs aimed at helping folks regain independence. People have left reviews about their positive experiences both online and in person, and interestingly, while the overall rating from some sources is 2.4 out of 5 based on a handful of reviews, the Google Reviews average is much higher at 4.8 stars, based on feedback from clients, families, and team members. They've got community photos online, a Facebook page, and a virtual tour for those who want to see the rooms and setup before a visit, which makes it easier for families who live far away or can't drop by in person.

Now, it's also fair to say the facility hasn't had a spotless record because the latest inspection, dated March 21, 2024, shows a history of 11 total deficiencies, including two related to infection control and one about food palatability and quality, with the cited tags being F0880 for infection prevention and F0804 for dietary issues. These are matters people looking for a nursing facility may want to review, especially if safety and meals are important to them. Still, folks who stay here generally get personalized healthcare plans, and the center's main focus stays on nursing and rehabilitation, while the common areas and amenities are designed for resident comfort and support during recovery. They hold a reputation for being a place that helps improve the quality of life, and that's been the case for many who walk through their doors, but just like any facility, it comes with its strengths and a few areas for improvement, and there's information and tours available for those wanting to learn more.
