Brandenburg Nursing & Rehabilitation Center sits as a for-profit limited liability company and offers both long-term care and short-term rehabilitation with 57 certified beds and an average of 55 residents daily, which keeps things more moderate in size so staff can know the residents but it's still a busy place, and you'll find that licensed nurses are available on-site at all times, day or night, taking care of folks by helping with daily living tasks, giving medication, and providing things like wound care, disease management, early intervention, vision and dental care, and restorative rehabilitation, and while the nurse turnover rate is 43.1%, which is actually a bit lower than what you might find in other places across the state, the nurse hours per resident each day run at about 3.49, which falls under the Kentucky average of 3.9, but they do their best with what they've got. Management's been under the oversight of Moshe Kelman since 2018 and Thomas Tackett since 2022, and ownership includes Benjamin Landa, Goldie Platschek, Alexander Platschek, and David Rubenstein, which is good to know if you like to keep track of those things. Now, some areas need work-for example, they've been cited for deficiencies in resident rights, safety, keeping the place clean and home-like, and especially nutrition, since they've had problems with food storage, where it comes from, preparation, and serving, and also compliance with care standards got flagged in reports. Even so, the center says they're committed to admitting residents without discrimination and to making things comfortable, plus there's a hospital readmission reduction program in place, individualized care plans, social activities, housekeeping and laundry, dietary counseling, rehabilitation for people recovering from surgery or illness, help with medications, and a Facebook page folks or families can check for updates, which might help you keep an eye on things happening around there. Still, there's no detailed information about the rooms or any fancy programs, but it's clear they handle most nursing and rehab needs and keep up with basic amenities you expect to see in a healthcare facility like this.

Caring staff but unsafe management
I love the caregivers here - kind, compassionate, and genuinely treat residents like family; the building is clean and feels home-like. Unfortunately I experienced serious clinical and management failures: medication and lab errors, limited bathing access, instances of neglect (mattress on the floor, poor attention), and very poor food. Because of those safety/leadership problems I cannot confidently recommend placing a loved one here until care and management improve.
