The place offers assisted living and intermediate care facility services, focusing on both medical and daily living needs, and three community ICF homes called Del Maria, Windsong, and Meadows give residents a personalized, homelike setting. Hazelwood Center covers everything from medication management and daily living help-like bathing, dressing, and transfers-to nutritious meals with restaurant-style dining, allergy-sensitive choices, diabetes diets, and a chef who makes sure there are meals all day. The rooms have private bathrooms, cable TV, Wi-Fi, phones, kitchenettes, and air conditioning, and residents can use areas like the library, game room, theater, activity room, garden, spa, sauna, walking paths, fitness room, and wellness space, all while enjoying landscaped outdoor spaces.

    The Hazelwood Specialty Clinic at the center takes care of complex health needs, giving residents and the community access to primary care, dentistry, psychiatry, neurology, ophthalmology, audiology, and behavioral health, along with therapy services like physical, occupational, and speech therapy, plus wound care, nutritional guidance, and psychological and behavioral management.
