    Excellent staff, systemic safety concerns

I had a mixed experience. The caregivers, therapists, and administrators I dealt with were compassionate, knowledgeable, and went above and beyond - the facility felt clean, welcoming, and great for rehab. However, I also saw or heard about serious safety and management issues (pests/mold reports, medication and handoff lapses, pressure-sore/incident concerns, poor communication and fear of retaliation), so while the staff is excellent, I'm cautious about fully recommending this place until those systemic problems are resolved.

About Maple Grove Senior Living, LLC

Maple Grove Senior Living, LLC sits in the countryside around Shelbyville, Kentucky, with a stately front porch, gazebos, courtyards, and beautifully landscaped outdoor areas giving it a peaceful feel, while the building's recent renovations help make things more comfortable and bring the residents together. This place is a non-profit skilled nursing home, licensed for 117 beds, with about 80 people living there every day. The staff, ranging from 51 to 200 employees, includes experienced professionals, therapists, and skilled nursing care professionals who work to meet both medical and emotional needs, which is something you do notice-since many stay long-term, there's a sense of familiarity here. Nurse aides provide about 2 hours and 37 minutes of care per resident daily, with licensed nurses averaging 4 hours and 33 minutes per resident each day, which is well above the Kentucky average, and nurse turnover's also a bit lower than in most places. You see about 3 minutes of physical therapy per person each day, so therapy's available though not for long periods.

Maple Grove has won several awards for long-term care and's recognized as the most awarded senior living and long-term care facility in Kentucky, plus it sticks close to the old Masonic tradition in its values. It's not joined to a hospital or a big retirement community, so it stays a bit quieter and more focused, which matches well with what people look for in the area. The building passed its last fire inspection in July 2024, has had no Medicare payment denials or federal fines in three years, and hasn't gotten complaints that led to citations. The most recent inspection showed four deficiencies-which is about average-including a couple for infection control and one about food sourcing, but there was no actual harm, just the potential for things to get worse if not fixed. It has a health inspections quality rating of 4, with an overall rating of 4 out of 5, but for resident care quality the score's around 2 out of 5 for short-stay and 3 for long-stay residents, which is something to think about.

The community offers several kinds of care: skilled nursing, long-term stays, assisted and personal care, respite services if someone stops in for a short stay, rehab services, and memory care programs for people who need help with memory loss. Residents and families can join councils to share their thoughts and help with decisions. There are activities to keep people engaged, and the place encourages connection and trust, trying to make it homelike as much as possible. Maple Grove is nonprofit and privately held and puts an emphasis on compassionate care and community involvement. For those who'd like to look around before coming, it features a virtual tour and photos of the community. Overall, Maple Grove Senior Living, LLC focuses on steady care, safety, and a sense of belonging, with a mix of practical support and attention to comfort, though it's smart to check what features and services are currently offered since there's not a lot of detailed information about them out there.
