Farmerville Nursing and Rehabilitation Center, LLC has 117 certified beds and sits under private ownership, split evenly between D&N, LLC and Dtd Hc, LLC, with indirect owners like Donald T Denz Irrv Tr and Norbert Bennett among others, and the place does focus on giving skilled nursing and rehab services so residents can get help with illness, surgery recovery, or injury, and the nursing staff put in about 4.32 hours a day per resident, which is more than the Louisiana average, though the nurse turnover rate does run high at 74.7%, where the state average's 48.4%, and over time, there have been 19 inspection deficiencies reported with some tied to safety issues, pressure ulcer care, and making sure staff know how to care for residents, and a complaint in January 2025 led to three new deficiencies and a fine of $30,258. Residents can get help with personal care, rehab after hospital stays, stroke care, memory and dementia support, palliative care, wound management, tracheostomy, and contracture management, and the facility offers therapy for physical, occupational, and speech needs, and has support programs for swallowing problems, pain management, incontinence, orthopedic injuries, and neurological conditions like MS or Parkinson's, even bringing in RT specialists for some procedures. There's mental health support, pet therapy, social work, pharmacy, dental, and IV therapy on hand, and outpatient services too, for folks who don't need to stay overnight, and the dining room offers five-star, restaurant-style meals while rooms come private or semi-private with TV, cable, Wi-Fi, and private restrooms. The Transitional Care Unit has its own private entrance for short-term rehab, and there's a big activity room open every day, a secured courtyard, and patios outside, making it easy to get fresh air, while indoors, seniors find new equipment for therapy and spaces for different group or quiet activities, and families looking for respite care can get some relief as well. The center is known in the area for kind care, and while it's Medicare and Medicaid certified and has various accreditations, it does work hard to tailor care to each resident, providing options like outpatient therapy five days a week, bariatric care, sports medicine programs, and support for different illnesses and disabilities, with the nursing and therapy staff holding certifications and years on the job, and for folks going through changes in health and care needs, the Transitional Care Center helps bridge the gap, always aiming for a home-like feel, even if it isn't perfect.

Nice grounds but poor management
I had a mixed stay. My room was nice, the grounds were well-kept with birds and deer, and activities/therapy were good - but management is poor, the facility is outdated and often dirty, and staffing is inadequate. I experienced missed meds, slow call-button responses and poor communication (even an unattended resident needing an ambulance); I can't recommend it.
