    Clean, welcoming facility; nursing issues

I had a mixed experience. The facility is clean, comfortable and welcoming - many staff were kind, attentive and helpful (great rehab, good food, high-tech check-in; special thanks to a few standout caregivers) and my loved one improved and felt at home. However, I also saw troubling nursing issues: unprofessional or underqualified nurses, delayed/denied pain meds and accusations, inconsistent rules/visitation, and occasional hygiene/care failures - serious red flags I recommend you investigate before deciding.

About Town & Country Health & Rehab

Town & Country Health & Rehab sits at 614 Weston St in Minden, Louisiana, and has provided nursing home and rehabilitation care since 1974, running as a family-owned facility under Mr. Walter Ledig with Ms. Elizabeth "Libby" Ledig as a secondary contact, and they offer skilled nursing and several therapy services for people of all ages, with long-term and short-term stays, including options for hospice, memory care, ventilator support, respite care, and rehabilitation focused on recovery after surgery, stroke, or accidents. The facility holds 143 skilled beds, including private and semi-private rooms, and handles complex medical needs, especially for wound care, ventilator-dependent patients, pediatric cases, and those needing technological support, and they've got a team including RNs, LPNs, and a Master Social Worker/Patient Advocate working seven days a week, with care plans built around each person using advice from doctors, therapists, and staff. They're known for IV therapy, wound care including vacuum therapy, specialized therapies for Alzheimer's disease, respiratory needs, whirlpool therapy, and regular physician visits from the Louisiana State University Health Sciences Center for pulmonary and critical care, and they accept Medicare, Medicaid, VA contracts, and several insurances. The place welcomes therapy dogs and pet therapy to help residents' moods, keeps an active events calendar with outings, has religious services from various denominations, and runs activities to keep folks involved, and there's even a beauty/barber shop and laundry service on-site for daily needs. Amenities aim to keep residents comfortable and at home, and they focus on safety through enhanced barrier precautions. The Inside Guide to America's Nursing Homes rates them a 5-star facility, and they show an A+ BBB rating, though they're not formally accredited by the BBB. Staff use therapy to help people regain mobility and strength, and every effort goes toward making sure residents heal, feel at home, and stay safe, with support designed for those with memory issues, end-of-life needs, or those hoping to return home after rehab.
