    Therapy great; facility unsafe, mismanaged

I have mixed feelings: the therapy team and CNAs are superb - my dad is happier, we visit more often, and Sadie the security cat is a warm, welcoming presence - but otherwise this place desperately needs help. Food and overall service are poor, management and teamwork are broken (high turnover, staff gossip and inaction), and I've seen unsafe incidents like wrong meds, a patient left on the floor, and unnecessary testing. Until leadership fixes these systemic issues (there are even reports of racist nursing behavior), I think it should be closed or completely overhauled.

    About Ringgold Nurse Care Center

Ringgold Nurse Care Center sits on Kenneth Street in Ringgold, Louisiana, and folks tend to talk about the place when they're looking for senior living that covers quite a few bases, since the facility's got 112 certified beds and usually cares for about 91 residents a day, and the building's a one-story setup where people can pick a private or semi-private room, each with space for personal things, cable, and a phone line to stay in touch. The center provides a lot of levels of care-nursing home care, assisted living, and memory care-so whether someone needs a little help with dressing, bathing, or medications, or needs round-the-clock skilled nursing, this place tries to cover that, and they've also got a Special Care Unit with a 40-bed capacity that's designed for people diagnosed with mental illness or behavior disorders, with 24-hour medical and psychiatric care, regular medication checks, and staff trained for complex needs, plus a secure courtyard attached for fresh air and safety.

Ringgold Nurse Care Center gets inspected by federal agencies like CMS and is known to be affiliated with D&N, LLC as part of its for-profit structure, and when you look back through public records, the place has had at least two complaint reports in the years 2024 and 2025 and some deficiencies found in recent inspections, especially on resident rights and abuse prevention, so people keep a close eye on the rules and quality reports. The ownership's split, with D & N, LLC and DTD HC, LLC each holding half, and there are indirect owners listed in records as Donald T Denz Irrv Tr and Norbert A Bennett Irrv Tr Fbo Children and Grandchildren, so it's got some layers when it comes to who runs things.

The nurse turnover rate is 52.4%, which is higher than state average, and each resident gets about 3.51 nurse hours a day measured out, so sometimes folks look at those numbers to get a sense of staffing consistency. The therapy side's pretty extensive here, with physical, occupational, and speech therapy available five days a week, whether you need rehab after a surgery, care for stroke recovery, pain management, or training for balance and falls, plus they offer orthopedic, neurological, and pediatric therapies, as well as help for conditions like Parkinson's, MS, and cerebral palsy. There's outpatient services too, for when someone doesn't need to stay overnight.

Rehab Suites are set up for short-term and long-term stays, focusing on complex cases post-hospital, like orthopedic injuries, cardiac illnesses, pulmonary issues, using modern therapy tools and plans made for each person, and a big part of the program's working on recovery from surgeries like hip or knee replacements, and a lot of folks use the gym, activity room, and outdoor courtyard while they're here. The staff takes on wound care, IV therapy, contracture management, urinary incontinence care, and managing tubes like tracheostomies, so people facing medical challenges usually get an individualized plan.

Meals on Wheels runs through the center, and there's a big list of support services-caregiver respite, personal care, homemaker program, recreation like singing or themed events throughout the week, and outings for holidays or special days, plus a beauty shop, housekeeping, laundry, and wheelchair-accessible transportation, so day-to-day life stays busy. They try to keep residents' rights in focus, too, with an Ombudsman program that's part of their process, and people can look at facility history, mission statements, council and board lists, and sometimes join the mailing list or volunteer.

Ringgold Nurse Care Center is listed as a nursing home in inspectable databases and directory categories, so you can look up their inspection results and see the past reports online, and if someone wants to see the place, there are tour options to meet the staff, see the facilities, and decide if it's a good fit. Even though there have been notes about deficiencies and turnover, the team keeps working on adapting care plans, offering therapies, and supporting people with medical and personal needs in a setting that feels homey but also structured for the level of help that's required.
