About Borderview Rehabilitation & Living Center

Borderview Rehabilitation & Living Center is a 108-bed facility nestled in Van Buren, also known as the “Gateway Town” to the scenic St. John Valley. This center is dedicated to meeting the full continuum of long-term care needs for individuals, recognizing that everyone’s needs can shift over time. To accommodate these changes, Borderview offers four distinct options of care, each designed to support residents in achieving the highest possible quality of life and independence.

The Skilled Nursing & Rehabilitation Service at Borderview provides a broad range of services aimed at helping individuals regain their independence following illness, injury, or surgery. The professional nursing staff is highly trained in specialized care, including IV therapy, wound care, pain management, and patient education. In addition, the therapy department at Borderview offers Physical, Occupational, and Speech Therapy services, available up to six days a week, ensuring that residents receive comprehensive rehabilitative support in a timely and effective manner.

Nursing Care at Borderview offers an extensive level of assistance for those who require ongoing health support. The primary goal of this service is to help residents maintain their independence to the greatest extent possible, while ensuring that their daily needs are met in a safe and supportive environment. Many residents come to consider Borderview their home, thanks to the center’s warm atmosphere and commitment to individualized care.

For those who require less intensive support, Sub-Nursing Care provides assistance with activities of daily living without necessitating the full range of medical services included in traditional Nursing Care. This option is ideal for individuals who can manage most of their day-to-day needs but benefit from some assistance to maintain their independence and well-being.

Residential Care at Borderview allows residents the freedom to live their lives on their own terms while benefiting from 24-hour supervision, a rich array of activities, and personalized care planning. This approach is designed to encourage choice, independence, and the creation of a true home-like environment for all residents.

Comfort and convenience are important at Borderview Rehabilitation & Living Center. Each room is equipped with cable television, with internet access and private phone lines available in most rooms, fostering communication and connectivity for residents and their loved ones. Family involvement is deeply valued, with staff arranging regular meetings and care plan discussions, and always prioritizing open feedback and communication. Many residents come to see the caregivers as an extension of their own families, reflecting the deep sense of community and trust that Borderview strives to cultivate. The center’s overall commitment is to service, dignity, and supporting each resident in their journey, making it a place where residents and staff alike feel at home.
