    Excellent daytime care, troubling nights

I had a mixed stay: the day team - nurses, therapists and administration - were kind, skilled and genuinely helped my recovery, and I'd recommend them. However I also saw serious lapses: abusive night shifts, residents left in bed, vulgar staff gossip, some dirtiness and people screaming for help, which made it feel poor value for money. Food and facilities were adequate, activities limited, so overall outstanding care at times but worrying neglect at others.

    About Alliance Health at Braintree

Alliance Health at Braintree has 101 certified beds, including a 19-bed Transitional Care Unit, and people usually see it as a skilled nursing facility that cares for both short-term and long-term residents, and the building has been managed by Alliance Health Management Services LLC since August of 2013 under the broader Alliance Health & Human Services group. The staff provide 4.73 nursing hours per resident each day, so that means residents generally see nurses more often here than at many other places in Massachusetts, and the nurse turnover rate sits at 30.2%, which is lower than the state average. The center's non-profit, owner-operated model focuses on a holistic, patient and family-centered approach, and the facility's team creates customized plans for residents dealing with different medical complications or working toward greater independence, offering help with post-acute care, rehabilitation, respite, palliative, and end-of-life care, and offering case management, activities programming, and social services to round out the care experience. The center holds accreditations from The Joint Commission, and it's also Medicare-certified with the CMS 4-Star rating, as well as skilled nursing certifications from MA and US DHHS-#225769 and #0445, for those who pay attention to those things. Those who need specialized medical care can get services like wound care, high flow O2, tracheostomy support, diabetes management, feeding support, pain management, orthopedic, cardiac, stroke recovery, and psychiatric care, with in-house therapists providing physical, occupational, and speech therapy, and other clinical services offered as needed, like IV therapy, EKGs, lab work, or modified barium swallow studies. Staff attend to residents 24/7, running everything from daily skilled nursing to more advanced treatments, and onsite equipment like in-wall oxygen helps with pulmonary needs. Recent surveys show 16 deficiencies, two of them related to infection, and others called out issues with food sourcing and handling, as well as small misses in care plan development, though the facility remains in compliance with federal guidelines for infection control and continues to participate in programs to fight flu, prevent sepsis, and discourage unnecessary antipsychotic use. The place also offers a van for resident transport, outpatient therapy, and has ongoing renovations-about $6 million going into updates since Fall 2023. There are education and support resources, including newsletters, caregiver training, webinars, and advocacy programs to support resident rights and staff, while participation in the Massachusetts Senior Care Association links the community into wider improvement initiatives. People use the Post Acute Unit after a hospital stay, and those needing longer-term care get a range of therapies or custodial support, and the staff work with residents and their families to help the transition between home, hospital, and skilled nursing care, always trying to improve the resident experience and safety with falls prevention programs and cultural change efforts. The size of the center means it handles a wide range of health needs in one place, and while Alliance Health at Braintree works to keep up with changing standards and upgrades, the staff focus on resident dignity, health, and comfort every step of the way.
