Island Terrace Nursing Home, which used to be open at 57 Long Point Road in the North Lakeville neighborhood of Lakeville, MA, served seniors who needed different levels of care, so if someone was looking for a place for memory care, skilled nursing, or even independent living with a few extra hands around, they welcomed a mix of residents for short or long stays even offering respite care for families who needed a break, with the Tolles family running things and treating people pretty well, or so folk said. With room for 77 residents, the place had neat features like an outdoor garden, a dining hall, a computer room, and a restaurant where people could sit for meals that had won some awards for being actually pleasant and nutritious, which can be rare, plus sweeping gardens, wildflowers, sunsets through the trees, and woodland paths, though you wouldn't find pets there, but you'd see plenty of birds since it's a wildlife sanctuary right on the water with acres of tended grounds, tucked into cranberry bogs and giving a bit of that country inn feeling. They provided help with personal needs like bathing and medicine, had nurses on staff at all hours, managed pain and vision problems, and could bring in psychiatry if needed, so if someone wasn't safe living alone or needed regular checking on, especially with Alzheimer's or dementia, they stayed attentive and made sure people didn't wander or get confused, and always had staff about for emergencies, care, or just conversation, and tours happened with both current residents and friendly staff around to talk and answer questions. Social activities got some attention, with choices like arts and crafts or spiritual groups, and people seemed to genuinely get involved, probably helped along by a culture known for being joyful and kind, which, along with the meals and activity programs, had earned the place a few community awards. They took various payment options-insurance, Medicare, Medicaid included-and fit the care to each person, whether someone needed full nursing, supported living, rehab, or just wanted to try out the independent living with some support in a simpler, quieter setting, though no new residents can move in now since Island Terrace Nursing Home is permanently closed, but while it was running, reviews were high, with an average score of 9.2 from 21 reviews, and the website at http://www.islandterrace.com still shares the look of the place and a bit about its history as part of a small network of care homes.

Poor communication, then outstanding care
I had a mixed experience. Early on communication was poor - doctors and the head nurse ignored our concerns, nurses wouldn't assist after signing out, my mom was signed out against her will and left without bathroom help, and as her healthcare proxy I was initially denied records and had to push for an ER visit. After readmission for CHF the care was outstanding: attentive, professional, loving staff, strong leadership, many activities, family-owned with owners on site and Christian values - they treated my mom like family and I felt she was safe. I highly recommend this five-star nursing home.
