    Inconsistent care, safety, hygiene concerns

I had a mixed, ultimately troubling stay. Some nurses and therapists (Paula, Cheryl, Reyna, Maryann) were caring, skilled and helpful, and rehab/activities helped some residents - but staffing was inconsistent and often inadequate. I experienced slow or missed care (meds, diapers, PT), poor communication, rude staff at times, and safety lapses that contributed to falls and hospital visits. Cleanliness, meals and maintenance were hit-or-miss - neat and warm on good days, but cold/unappetizing food, hygiene concerns, and broken fixtures on others. Because of the care and safety issues, I wouldn't trust this place for high-need elders.

    About Bear Mountain at Reading

Bear Mountain at Reading sits at 1364 Main St, Reading, MA, and provides many services for older adults who need care, help, or recovery before going home, and you'll find a mix of short-term rehabilitation and long-term skilled nursing under one roof, with the facility set up for frail residents who need someone checking on them around the clock. Folks can choose from private or semi-private rooms, bringing in personal belongings to make their space feel homey, with both friends and family able to visit, and even pets are welcome, which eases things for many. Bear Mountain has Pavilion Suites-hotel-like rooms for those coming from the hospital and needing a short stay-and there's also "The Gym," a rehab center with modern equipment and plenty of therapy options for those working to get stronger. The staff covers a lot: occupational therapy, speech and language support, even neuro-rehab for people with brain injuries, and a nursing team focuses mostly on helping residents regain independence and go home, but they also provide complex care like wound management, stroke rehabilitation, and advanced IV therapy. Bear Mountain at Reading offers memory care, assisted and independent living setups, adult care home options, short-term and long-term care, plus respite for caregivers and end-of-life support if needed, but right now they aren't accepting new patients, which is important for families to know. Meals are made with nutrition and individual diets in mind, overseen by a registered dietitian, and there's a private dining room if you want to eat with visitors, who can also buy guest meals, while cable, WiFi, flat-screen TVs, and a phone system help keep residents connected. Residents can get their hair done in the Beauty/Barber Shop, use laundry services, and join in regular activities like crafts, educational talks, spiritual groups, and social events, with support from a social service team who help families with transitions and offer counseling as needed. Doctors and therapists handle post-surgical care, pain management, diabetic care, wound care, chest tubes, ostomy needs, and nutritional counseling, and the staff takes part in safety programs like Sepsis Smart and Falls Prevention, while focusing on culture change to improve daily life for everyone. The facility stays open day and night, has an emergency system in every room for safety, and the grounds are landscaped with areas to relax outdoors, because fresh air can help with recovery and mood. Ratings are mixed, with a 3.1 average from nine reviews, but the team tries to keep things clean, calm, and welcoming, always aiming for attentive care focused on the person's health plan, changing care as needs shift, and looking out for both residents and their families. Staff use English to communicate, follow training standards, and work continuously to keep up with quality initiatives and public health campaigns, offering guidance for everything from family caregiving to support and referrals. Those looking for a straightforward, wide range of senior care services will find Bear Mountain at Reading covers nearly every option, from the very frail to the short-term rehab patient, and the overall approach is steady, professional, and focused on helping folks feel as comfortable as possible for as long as they stay.
