    Neglect, mold, lies, ruined care

I would not recommend this facility. It seemed OK at first and might suit some long-term needs, but my experience was horrible: management behaved like slumlords, basement mold, ongoing heat/AC cutoffs, burnt-out staff and declining care that worsened my loved one's health. I was lied to, had items go missing and felt conned; I reported everything to APS and LARA.

    About Creekside Manor

Creekside Manor is a care home that provides living arrangements for individuals with special needs, offering a variety of residential options on its property. The facility is operated by Mike Chafe and his team, and it appears to focus on residents who require different levels of care, including those who need medication management and frequent wellness checks. Among its accommodations is an Adult Foster Care (AFC) home, which seems to be the initial living environment for some residents before transitioning them to more independent settings.

One of the independent living arrangements at Creekside Manor includes a three-bedroom apartment that is listed as a single-family home with two bedrooms. The actual use of the property is as a three-bedroom unit designated for residents, although the specific services and degree of independence vary based on each individual's needs. Another option provided by Creekside Manor is a two-bedroom trailer that is also listed as a single-family home. This trailer has a divided layout, sometimes referred to as a "duplex," with separate entrances for each resident. For example, one resident may use the back door while the neighbor uses the front, enhancing a sense of privacy and separation within the living space.

The home is managed and overseen by Mike Chafe, who is responsible for coordinating care, handling medication delivery, ensuring routine check-ins, and attending to property maintenance. Air conditioning and heating amenities may vary by unit, and the trailers and apartments have their own heating and cooling systems. Residents may bring in additional items, such as screen windows and fans, to make their stay more comfortable. The environment is intended to balance independent living with adequate supervision, tailored to meet the practical and health needs of each resident.

Creekside Manor also works with guardians and families of residents in managing day-to-day care and major decisions affecting residents. In some cases, guardianship may be transferred or closely monitored, and families might play a significant role in coordinating care alongside the operators. The facility has established procedures around moving residents between different types of accommodation based on their level of independence or specialized care required.

The operations at Creekside Manor extend into property management, with ongoing communication between residents, their guardians, and the home’s management to address maintenance needs and suitability of living arrangements. The property’s layout includes both apartment-style residences and modular housing options, aiming to provide flexibility for a range of care needs within a consolidated campus. Creekside Manor remains focused on accommodating adults with special requirements through individualized care and housing solutions under the oversight of Mike Chafe and his associates.
