Established in partnership with KCMHSAS, Transitions of Kalamazoo offers a unique model that is both intensive and transitional, serving individuals who have experienced challenges in other community settings or may be at risk of psychiatric hospitalization. The home itself is structured as a duplex, with distinct North and South units. Each side features three single bedrooms that offer privacy and comfort to residents, while a shared vestibule provides a welcoming common area with an inviting view of the backyard, fostering a sense of community in a homelike environment.

    The program at Transitions of Kalamazoo emphasizes individualized care through a combination of experienced staff and specialized training. Staff members are present 24 hours a day, ensuring round-the-clock support and supervision. Regular staff meetings are held to address and support clinical challenges as well as to maintain consistency in the implementation of each individual’s care plan. In addition to the direct care team, both a Program Manager and a Clinical Director are on site, adding further layers of expertise and oversight to support both staff and consumers.
