They've got a care team made up of nurses, doctors, social workers, aides, and therapists, and the team works closely with both in-house staff and some independent contractors, always focusing on the well-being and goals of each participant, and they even help people figure out if they qualify for Long Term Care Medicaid and walk them through the paperwork.

    The program includes everything ranging from medical visits, prescriptions, therapy, and durable medical equipment, to home modifications like wheelchair ramps, and even urgent or emergency medical care, because sometimes the little things like help with bathing, cleaning, grocery shopping, or medication management add up and folks need support with those too, so you've got services like in-home personal care, homemaking, meal delivery, prescription drop-offs, and aids who will assist with errands or household chores.
