    Excellent rehab, concerning safety lapses

    I found the staff overwhelmingly compassionate, friendly and expert - rehab/PT/OT was excellent, the facility clean, admission/discharge well coordinated, and meals (including vegetarian) satisfactory. However, I experienced troubling safety and communication lapses: restricted family involvement, poor incident reporting and nighttime monitoring problems that preceded an unwitnessed fall and hospital transfer. Great for rehab and kind caregivers, but verify safety/communication practices before entrusting a vulnerable loved one.
