Accra Care XIII is dedicated to enhancing the lives of individuals in need of homecare services by offering a wide variety of personalized care and support options. The organization recognizes that each person’s situation is unique and takes pride in providing individualized care within the comfort and familiarity of their own home. Services deliver assistance with the core activities of daily living for both adults and children, including dressing, grooming, eating, bathing, mobility, transfers, positioning, toileting, and other personal needs. For adults, support extends to light housekeeping, laundry, and meal preparation, ensuring a comprehensive approach to home-based care that fosters dignity and independence.
A distinctive aspect of Accra Care XIII is its focus on empowering clients through self-directed services. Personal Care Assistance (PCA) Choice services enable individuals and their families to take an active role in directing their own care. This level of involvement allows clients to select trusted, qualified caregivers who are best suited to meet their specific needs and preferences. Through Financial Management Services, clients who participate in programs like Consumer Directed Community Supports (CDCS) or the Consumer Support Grant (CSG) are given the flexibility to hire caregivers and secure additional supports essential to remaining in their community. Accra Care XIII offers assistance with the employment of support workers and the purchase of necessary services, making the process as straightforward as possible for clients and their families.
Comprehensive support includes services licensed under the 245D designation, such as respite care, homemaker services, Individual Community Living Support (ICLS), Individual Home Supports (IHS) without training, IHS with training, and night supervision. For each service, clients can select their own support staff, while for certain programs like IHS with training, Accra Care XIII ensures a qualified match is made to address the client’s unique needs. Each client benefits from the ongoing guidance of a dedicated Accra Service Coordinator, who collaborates with the client and their county case manager to develop and administer a care plan that fully addresses the individual’s goals and requirements.
The reach of Accra Care XIII extends throughout Minnesota, providing homecare to people of all ages—from children to older adults—in all 87 counties.
