Accra Home Care is a leading provider of individualized home care services throughout Minnesota, offering support that empowers people to live independently in their own homes and communities. With a mission to improve lives, Accra serves children, adolescents, adults, and families who require assistance for a loved one with a disability or for an older adult. The organization is recognized as the largest provider of home care services in the state, serving more than 11,000 clients across all 87 Minnesota counties, and employing over 16,000 people in both direct care and administrative roles.
The range of services at Accra Home Care is extensive, with a strong focus on individualized support. Personal Care Assistance (PCA) services are available to both children and adults, designed to help with the activities of daily living including dressing, grooming, eating, bathing, transfers, mobility, positioning, and toileting. For adults, these services also include light housekeeping, laundry, and meal preparation, ensuring that essential daily needs are met with compassion and professionalism. PCA services are flexible and can be provided both in the home and the wider community, empowering clients to live as independently as possible.
Accra offers a unique approach through its Financial Management Services, assisting clients with long-term care needs or disabilities to easily navigate self-directed service options such as Consumer Directed Community Supports (CDCS) or the Consumer Support Grant (CSG). Through this approach, clients can hire and employ caregivers of their choice or purchase other supports that help them continue living in their communities. In these self-directed programs, Accra provides the guidance and infrastructure needed for clients to manage their care efficiently and confidently.
In addition, Accra is a 245D licensed provider of a variety of services, including respite, homemaker, Individual Community Living Support (ICLS), Individual Home Supports (IHS) without training, IHS with training, and night supervision services. The care provided in these programs is highly personalized: clients can select trusted, qualified workers who best fit their individual needs for each 245D service, while IHS with training pairs clients with staff specifically matched to their support requirements. Throughout each client’s care journey, an assigned Accra Service Coordinator works closely with the client and their county case manager to set up and administer all 245D Waivered Services, ensuring that each support plan is tailored and comprehensive.
Accra Home Care takes pride in fostering a personalized approach that supports the independence and dignity of every client. The organization assists families by helping them navigate the various choices and service pathways available. Existing clients have access to a job board, enabling them to find caregivers who are the best fit for their needs, further reinforcing the sense of choice and individualized care that defines Accra’s philosophy. The organization’s commitment is evident in every aspect of its service, making a positive difference in the lives of those they support and strengthening families and communities across Minnesota.
