This is made possible through programs such as Consumer Directed Community Supports (CDCS) and the Consumer Support Grant (CSG), which empower clients to remain engaged and independent within their own communities. Accra Care VI also aids clients in employing support workers and managing the purchase of other vital supports necessary to maintain their lifestyle in the community.
Accra Care VI holds a 245D license, offering a comprehensive range of services that include respite care, homemaker services, Individual Community Living Support (ICLS), Individual Home Supports (IHS) with and without training, and night supervision services. For clients requiring IHS with training, staff members with the appropriate qualifications are matched to best meet individual needs, while for other 245D services, clients are able to select their own trusted and qualified workers. The care model includes the support of an assigned Service Coordinator from Accra Care VI, who works in partnership with county case managers to facilitate and administer the various 245D Waivered Services, ensuring a smooth and well-coordinated provision of care.
