The Congregate Housing Services Program (CHSP) at St. Paul Public Housing Agency is designed to support elderly and disabled residents in maintaining their independence for as long as possible. This individualized program caters to those who require a moderate level of non-medical services in order to continue living on their own. CHSP operates within two designated hi-rise communities, the Montreal Hi-Rise and Ravoux Hi-Rise, each of which is equipped with a CHSP Coordinator. The coordinator plays a central role, providing personalized support to residents by assisting with information and referral services, arranging transportation, setting up appointments, and making necessary connections with community resources. The coordinator and staff work closely with each resident and their care teams to ensure that unique needs are met on an ongoing basis, including daily observation of participants to address any emerging concerns promptly.
Nutrition and healthy living are essential pillars of the CHSP philosophy. Recognizing the importance of good nutrition, the program provides meal services that include one meal per day from Monday through Friday and two meals per day on weekends. These meals are served in a congregate dining setting in the hi-rise community room, fostering a sense of community and encouraging social interaction among residents. Additionally, for those eligible, a second weekday meal may be available in collaboration with the Title III meal provider. Beyond nutrition, CHSP ensures that residents benefit from regular housekeeping assistance and laundry services tailored to individual needs and abilities.
