    Compassionate staff, inconsistent unsafe leadership

    I have mixed, painful experiences with this facility. On the plus side many CNAs, nurses and PTs were compassionate, attentive, and went above and beyond - therapy helped my mom walk again, activities and outdoor space were appreciated, and some staff treated residents with dignity. But overall leadership and staffing are inconsistent and sometimes unsafe: ignored call lights, long waits, missed or wrong meds, poor hygiene, privacy/HIPAA breaches, terrible food, bruising and injuries, and unresponsive administration - my family transferred my father after prior neglect and I believe disorganization and neglect contributed to his passing. Communication is spotty and management needs major accountability; the frontline team can be excellent, but the facility as a whole is unreliable and I cannot fully recommend it without clear improvements.
