Aspire of East Prairie sits at 186 Millar Ave. in East Prairie, Missouri, and serves about 28 residents on average, though it has 70 certified beds, and the building is owned by Chp Snf Opco Holdings LLC, a for-profit company, so you get all the basics for older adults who need help with daily life, like assisted living, memory care, skilled nursing, adult care home, and even independent living for those who want a bit more freedom, plus there's a focus on supporting seniors who need short-term rehab or hospice care, and nurses spend about 3.77 hours per resident per day, which is better than the state average, though with a nurse turnover rate at 51.4%, it's less than Missouri's average but still something you notice over time, and Medicare and Medicaid are both accepted here which helps people who need financial help, and there are personalized care plans aimed at helping with everyday tasks so folks can get the right amount of support without feeling like they're just a number, and there are home health and home care services woven into the mix along with medical supervision by experienced doctors, while the amenities include a variety of studio room options and community activities that try to help people feel connected and less alone, with support services and recreational programs that do their best to build a sense of belonging. Aspire of East Prairie has a 3-star rating from the Centers for Medicare & Medicaid Services for its nursing home, and recent reports show 29 total deficiencies, including five related to infection, issues with keeping the place clean, keeping up resident rights, and meeting nutrition and dietary standards-reports mention food coming from unapproved sources and trouble with safely getting rid of garbage, so the building has room for improvement when it comes to making things homelike and safe, which is pretty important to some folks. Community reviews average about 5.8 out of 10, so people have mixed feelings, but the focus at Aspire tends to be on nurturing relationships, giving companionship, and designing plans for each resident's needs, and activities are made for older people to promote a better quality of life, while the range of medical care tries to serve anyone from those who are still pretty independent to those who are very frail or need memory care. The facility carries Aspire Senior Living's name so there's a larger network behind it, and the staff tries to help residents with both medical and daily needs, whether that's through skilled nursing, memory care, or more everyday support, and families can expect a range of services aimed at keeping aging adults cared for, socialized, and looked after, though the inspection reports show there can be challenges along the way, just the same as you'd find in many places that care for elderly folks.

Elder neglect, understaffed, abusive management
I trusted this facility with my grandmother and was horrified. Some staff showed care and a positive attitude, but chronic understaffing and poor teamwork led to neglect: she went five days without eating, was covered in feces, unbathed, and hadn't had her teeth brushed in over a week. Staff were rude, I was even denied access when she was dying, and I believe I witnessed elder abuse. The administrator should be fired and the facility should be shut down unless immediate, serious improvements are made.
