    Poor communication, unsanitary, obstructive care

    I placed my mom here and it was the worst decision - poor communication, unresponsive staff/management, visitation restrictions that kept me from seeing her, and emergency contact changes without consent. The building was unsanitary (vomit in the sink, no water fountains, hydration concerns), residents were sick and I was even denied transportation for a court date - I felt her release was obstructed and possibly illegally withheld. A few staff were kind and tried to advocate, but overall I'm relocating her closer to family and cannot recommend this facility.

    Levering Regional Health Care Center in Hannibal, Missouri, sits on Market Street and has been around since the original Levering Hospital started in 1903, with land and a building donated by Mr. and Mrs. A.R. Levering, and these days the facility's part of Reliant Care Management. The facility has 179 beds, both short-term rehab and long-term care, and also offers assisted living, skilled nursing, and memory care for those with Alzheimer's and dementia, including tailored care plans and secure areas for those who need extra protection. Folks there can get daily living help, medication management, bathing, dressing, transfers, IV therapy, tube feeding, post-op care, and personal laundry services, all with assistance available any time of day or night and a physician on call 24/7. The place keeps up with resident-run activities, full social calendars, and neighbors often form friendly bonds with each other through special events, educational activities, and visits to the library, secured courtyard, walking paths, garden, or on-site barber and salon. Residents can decorate their own rooms, use cable TV hookups if they want, and enjoy transportation to shopping, restaurants, parks, and medical visits nearby. Therapy and rehab happen in a modern department, offering a wide range of treatments, and dental, podiatrist, and eye care are on site, too. You'll sometimes see staff and residents described as caring and loving, and the place does encourage open communication and a team approach.

    Still, Levering Regional Health Care Center has had serious problems over time, including 59 care deficiencies by October 2023, three Medicare payment suspensions, $517,000 in penalties, and reports of failing to keep equipment and the environment sanitary or safe. Reports include failing to manage health needs, not giving needed treatments like dialysis or foot care, not helping with eating, letting wet ceiling tiles with mold remain, and not cleaning up after spills or soiled sheets and toilets. Staff have sometimes failed to review care plans, respect the residents, or investigate abuse claims, which means some families have felt let down. The building itself is roomy and well-lit, with large hallways, and tries to provide privacy and neighborly connections in a quiet neighborhood with shopping and parks nearby, but reviews and inspections have called out neglect and risks to residents' health and safety in the past.
