Meadow View Health & Rehabilitation sits at 2203 E Mechanic Street in Harrisonville, Missouri, and houses up to 120 residents in both private and semi-private rooms in a single-story design so nobody needs to climb stairs, and there's always skilled nursing care available around the clock whether someone's there for short-term rehab, long-term care, or to live in the 22-bed Alzheimer's secured unit with trained staff and locked exits for safety, and while some people find comfort in the activities like bingo, arts and crafts, church services, and other social events, there have been serious issues over the years, including times when staff made rude or inappropriate comments about residents' personal care or appearance, and staff have even tried to emotionally manipulate residents out of money or belongings, and there've been citations over residents with aggressive behavior hurting others with not enough steps taken to prevent it from happening again, plus a history of problems such as residents being left in hospital gowns instead of regular clothes, staff yelling around the dining room, and nurses not always following advanced care directives, which means folks sometimes didn't get their wishes honored, and there have been numerous failures to ensure residents' dignity or protect them from abuse and neglect or wrongful use of their things, so in total, the place has added up at least 56 documented deficiencies and paid $139,000 in fines from inspection reports, and offers medical care, meals, laundry, and transportation for those who need it, but with a nurse turnover rate of almost 59%-a bit higher than the state average-and fewer staff nurse hours per person each day than the state norm, many folks will want to look closely at all the records before picking this facility, because it does provide care for frail seniors who need a lot of help and has some nice activities for creative or social people, but one should be aware of its long-standing problems, including issues like food not always being served according to professional standards and therapy not always being up to expectations, and while the administrator is Amy Vandaveer and the place is operated by Reach LTC as a for-profit entity, people should know that Meadow View Health & Rehabilitation tries to create a setting where residents join a "special family" and get personalized care plans, but there's a clear history of significant safety and dignity concerns that show up in inspections, so it's best for people to consider both the range of care options and the documented record of this health and rehab facility before making any decisions.

Mixed care, neglect, hopeful staff
I found a mix: kind, hardworking caregivers and a sweet therapy dog, but also alarming neglect. Chronic understaffing, missed/late meds and IV problems, infrequent bathing, dirty/outdated rooms, safety incidents and unresponsive management made the stay emotionally traumatizing. New ownership and some excellent staff/therapy show promise, but I would not trust them for advanced memory or high-need care without daily oversight.
